Excellent Practice Consultant Position with a Leader in Healthcare Accountable Care Organizations (ACO) - multi-state provider based in Louisville, KY!

POSITION SUMMARY: A Practice Consultant provides operational support to the Imperium client services team, working to provide ground level tactical assistance to help ensure the launch and management of initiatives at client practices (ACO's). The candidate will play a support role working with provider groups towards improving quality scores, patient outcomes and fiscal performance.  The main goal of this position is to optimize health information management systems by integrating EHR's with physician decision support, population and patient-panel management tools, appointments, registration, and billing systems.

POSITION RESPONSIBILITIES

  1. Train physician practices on guidelines and on best practices

  2. Work in the physician practices to foster change faster

  3. Improve care coordination

  4. Assistance with Advancing Clinical Information (ACI), HCC, and GPRO

  5. Improve transitional care by working with physician practices on:

    1. Workflows

    2. Patient identification

    3. Billing

    4. Documentation

    5. Capacity and capabilities

  6. Monitor progress and performance

    1. Track initiative across the market

    2. Facilitate rapid process changes

    3. Monitor and report back about practice and provider on progress

    4. Identify and address gaps in care, access, and performance barriers

QUALIFICATIONS:

1.    Hands-on individual with great attention to detail, high personal accountability, and strong drive to develop him/herself while learning our business model.

2.    Bachelor's degree preferred.

3.    Healthcare experience desired.

4.    Strong organizational skills.

5.    Excellent written and verbal communication skills.

6.    Requires travel to local clients on daily basis and may require travel within the state.

7.    Knowledge of HEDIS, Medicare, and ACO practice management preferred.
