Vascular Access Team (VAT) Registered Nurse
Location: Kankakee, IL
The Vascular Access Team Nurse (VAT) is responsible for the safe delivery and documentation of all aspects of Vascular Access patient care. Under general direction the VAT nurse has responsibility and accountability for assessment, planning, implementation, and evaluation of vascular access sites (eg. PICC, Central Lines and IV sites) throughout the system. The VAT nurse serves as a clinical resource and consultant to other health care professionals in the field of Vascular Assess. The VAT nurse guides nursing personnel and collaborates with other healthcare professionals to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Uses knowledge of patient's age and cultural diversity into the provision of patient care.
Work Schedule: Varies
Accessing port-a-caths. Assessing patient for appropriate. Vascular Access based on GFR and long term care for patient. Central line surveillance. Consistently and appropriately analyzes the assessment data to determine the patient's nursing diagnoses or patient problems. Consistently and appropriately contributes to the professional development of peers and colleagues through effective interaction and mentoring in the planning and delivery of nursing services to achieve optimal outcomes. Consistently and appropriately detects and collects comprehensive data and identifies patterns pertinent to the patient's health and unique situation. Consistently and appropriately develops a plan that prescribes strategies and alternatives to attain expected outcomes, based on the patient's individualized situation and anticipated needs. Consistently and appropriately effects change through engagement, involvement, and participation of peers and colleagues, leading others where they need to be rather than where they want to go (American Nurses Credentialing Center [ANCC]). Consistently and appropriately implements the identified plan based on the nursing process, our nursing mission and philosophy, and our organization's mission. Cross-trains in Interventional Radiology. Dressing changes. Initiates appropriate education and instruction to patients/families, based upon evaluation to meet their learning needs. Insertion and care of IV's. Midline insertion, care and maintenance. Provides consultation, education, and/or assistance to staff regarding management of vascular access devices. Responds to codes to establish IV access. RN Peer Review Feedback. Save the Vein promotion, education, and surveillance. Sterile technique.
