CAC-Florida Medical Centers is seeking a Board Certified Endocrinologist. The candidate will enjoy providing high quality health care in a professional and caring environment to a primarily geriatric population and work in collaboration with the Diabetes Disease Management team. CAC-Florida Medical Centers has the presence of 11 medical centers throughout the greater Miami area and Homestead. Assignment Capsule


Practice medicine following standard of care and evidence-based medicine guidelines in order to guide patients towards positive health and well-being outcomes.


•     Has the necessary skills and appropriate licensure / certification to provide superior guidance at points along the clinical continuum.


•     Must be Board Certified or Eligible in the given specialty with the knowledge and skills to follow standard of care guidelines and maintain appropriate documentation. Recommend services for Humana Plan members utilizing care alternatives available within the community and nationally.


•     Examine clinical programs information to identify members for specific case management and / or disease management activities or interventions by utilizing established screening criteria.


•     Conduct admission review and post-discharge outpatient follow-up


•     Able to make appropriate patient assessments and provide guidance for plan of care. Advises on health maintenance and disease management programs. Understands medical utilization and management. Has experience working with a geriatric population


•     Understands and has experience with electronic medical records. Understands and follows medical record documentation guidelines.



•         Maintains a professional and collegial attitude towards patient population and staff. The ability to encourage and engaging in a team approach to health and wellness.

Qualifications


Role Essentials


•     Active Florida medical license


•     Current DEA license


•     Ability to be licensed in multiple states without restrictions


•     Board Certified or Board Eligible in given specialty


•     Bilingual – English and Spanish


•     Experience with electronic medical record documentation



Role Desirables


•     Advanced work experience in a hospital setting or private practice


•     Previous Medicare/Medicaid Experience a plus


•     Experience with eClinicalWorks EMR a plus



•         Creole and other languages are beneficial



Reporting Relationships


You will report to the regional medical director of CAC Florida Medical Centers.
