We are currently seeking qualified candidates for a Medical irector position that will be based in our Population Health department.  This position is based in the East Florida Region.
This position has the opportunity to be responsible for leading, guiding, and supporting the development and implementation of new clinical capabilities, protocols, and pathways consistent with Population Health Management across the East Florida Region provider system and associated physician networks.
Specific job duties include:
  • Serves as point of integration for various EFR PHM initiatives
  • Serves as Medical Director for Florida Care Network (separate job description)
  • Serves as Medical Director for Florida Memorial Health Network (separate job description)
  • Serves as a regional link to FHCA’s medical management department
  • Leads the development and adoption of cost effective clinical protocols for high cost/high utilization conditions across the EFR
  • Assist the provider system in developing programs to reduce inpatient utilization and avoid readmissions. Provides expert medical opinion and analysis that will contribute to establishing standards for quality assurance, disease management, complex case management, and chronic disease management.
  • Primary physician champion for all EFR PHM Activities
  • Counsels physicians, or meets with leaders of physician groups, on performance improvement with respect to PHM activities and targets including the adoption and implementation of quality and disease management metrics
  • Assists in the development of provider system-based disease management programs. Including setting performance metrics in care management and in issues relating to regulatory oversight impacting patient care
  • Assists the EFR executive leadership in setting and articulating the population health management vision and gaining commitment of internal and external providers
  • Provides medical leadership to the regional network operations team and assists in development of the overall EFR provider network strategy
  • Functions as a liaison for clinical issues between network providers and the administrative arms of the organization, including the FHCE, FCN, FMHN, and FH facilities
  • Serves as a reginal “connector” to PHSO activities, such as care management, and PHSO leadership 

Education Requirements:

  • M.D. / D.O. degree from an accredited program, required.

Experience Requirements:

  • Minimum of ten years of medical practice (includes training), required.
  • Minimum of five years clinical leadership experience in a health plan, large integrated medical group, or large healthcare delivery system
