We are currently seeking qualified candidates for a Population Management Educator position that will be based in our PHSO Provider Operations and Engagement department. This position is based in Park Ridge ,NC

The purpose of this position is to provide Population Health subject matter expertise, consultation and education to PHSO network providers (market/region dependent) by establishing and maintaining a strong relationship with the provider and practice while bringing education for specific Population Health related technologies. Population Management Educators are expected to make regular visits to provider practice offices as needed, with the intent to educate on a wide variety of topics (examples include population health management strategies and programs, clinical data integration, Electronic Health Records (EHRs), IBM Watson Health Explorys, provider web site, etc…). Contributes to preparation of training materials and provides education on new applications or functionality.  Conducts train-the-trainer, super-user, upgrade and/or other training with moderate direction from the application manager.  Ability to recommend education of the application for unique ambulatory provider/practice scenarios and build using application tools with assistance from team members and/or the application manager. Able to articulate complex educational topics in a way which facilitates understanding, both amongst providers as well as other PHSO or AIT team staff.  Uses knowledge to help connect key concepts and expectations, ultimately presenting the technologies, educational opportunities, and efficiencies in a manner that is simple for providers to relate to and brings value to the ambulatory practice.

Specific job duties include:
 
  • Demonstrates basic level functional knowledge of one or more of the assigned applications.
  • Contributes to preparation of training materials and provides education on new applications or functionality.  Conducts train-the-trainer, super-user, upgrade and/or other training with moderate direction from the application manager. 
  • Demonstrates basic knowledge of processes within the facility department.      
  • Ability to recommend education of the application for unique ambulatory provider/practice scenarios and build using application tools with assistance from team members and/or the application manager.
  • Participates in team meetings, project meetings, and other meetings when requested. 
  • Collaborates with hospital leadership, physicians, clinicians, end users, Adventist Information Technology (AIT), vendors and associates to develop and deploy electronic applications or new functionality with assistance from team members, the application manager and/or the project manager.
  • Actively participates in facility support through solution implementation, activation, and post-live support.
  • Able to articulate complex educational topics in a way which facilitates understanding, both amongst providers as well as other PHSO or AIT team staff. 
  • Uses knowledge to help connect key concepts and expectations, ultimately presenting the technologies, educational opportunities, and efficiencies in a manner that is simple for providers to relate to and brings value to the ambulatory practice.
  • Facilitates practice change management in regards to emerging trends related to population health initiatives, reimbursement models, patient satisfaction, customer outreach, panel growth, and provider performance with quality and efficiency metrics. The Educator should be seen as an asset to their practices by providing important information and recommended enhancements to help ensure success in the coming value-based healthcare environment with their healthcare technologies.
  • Responsible for tracking, measuring, analyzing, and reporting on provider contacts, updating provider demographics, validating provider contact information, and reporting issues in the designated provider reporting tool.
  • Works seamlessly, effectively, and in harmony with all internal teams including, but not limited to, PHSO credentialing, provider resolution center, quality and care integration, provider engagement optimization, analytics, the respective and finance to ensure integrated operations that result in a superior consumer experience for the provider.

Job Requirements

Education Requirements:
 
  • College Degree (AS, B.A., B.S. or similar) in a field of study such as business, IS, education, or a healthcare related area
 
Experience Requirements:
 
  • Yearly participation in ongoing education either formal or through conferences, active professional organization participation, or journal presentations.
  • 2+ years’ experience working collegially with physicians
  • eClinicalWorks - Preferred
