The Vice President of Post-Acute Care Services position is responsible for the all planning, development, implementation, and operational oversight and strategic development of Post-Acute Care services at Florida Hospital (FH) and reports to the Senior Executive Officer for Care Continuum. These businesses include FHs’ Home Health, Hospice of the Comforter, Home Infusion, Durable Medical Equipment, and Inpatient Rehabilitation. Further, this individual is responsible for external affiliations and or community partners that include, but not limited to, skilled nursing facilities and long-term acute care hospitals as part of the FH Post-Acute preferred provider network. This individual delivers overall strategic planning and direction for the aforementioned post-acute care services across the FH system and with key stakeholders to formulate new innovative approaches to care. In addition, the VP engages and collaborates with the FH division, and as directed by SEO, with AHS, on elevating quality and services related to post-acute care services. He/she will work with post-acute care providers, partners, physicians, and care management to ensure that care is coordinated across the continuum to allow for smooth transitions to the appropriate level of care and services. The VP works in a matrix organization and serves as the post-acute care executive ensuring each business provides consistent measures of success. Measures of success result in clinical, financial, and market outcomes that demonstrate value to patients, physicians, payors and the organization. He/she is accountable for achieving established outcomes through actively engaging interdisciplinary teams and external stakeholders. Participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Adheres to the Florida Hospital Corporate Compliance Plan and all rules and regulations of all applicable local, state and federal agencies and accredited bodies. The position must execute the short-term and long-term goals in a manner that is consistent with the mission, vision, and values of Florida Hospital and across the continuum.


Specific job duties include:
  • Provides strategic and operational leadership to Post-Acute Care (Inpatient Rehabilitation, FH Home Care, FH Hospice of the Comforter, FH Home Infusion, FH Respiratory & Equipment), all undertaken to improve business unit performance and sustainability, and to support the FH need for robust and Post-Acute Care access for patients.
  • Oversee FH Hospice of the Comforter Finance administrator, responsible for 290 FTE’s for this $23M net revenue business with a $25M expense budget.
  • Oversee FH Home Care Finance administrator, responsible for 141 FTE’s for this $12M net revenue business with an $11M expense budget.
  • Oversee Inpatient Rehabilitation administrator responsible for 65 FTE’s for this $24M net revenue business with a $19M expense budget.
  • Provide leadership to Post-Acute Joint ventures FH Respiratory Equipment and FH Home Infusion. These business units combine have net revenue of $17M and an expense budget $16M.
  • Evaluate and advise senior leadership on joint venture, partnership, and collaboration opportunities.
  • Launch the post-acute care service network, preferred provider network and related programs, ensuring that it meets the needs of patients served by Florida Hospital and achieves the aim, strategic goals, and outcomes of the organization.  
  • Provide leadership and direction for system, departments, services, clinical programs and support areas involved in the provision of post-acute care.
  • Effectively collaborates and communicates with caregivers, managers, physicians, and system leaders while developing, implementing, and operating the program.
  • Responsible for hiring and retaining qualified personnel through effective job design, recruitment, retention, development, and performance management programs and processes.
  • Responsible for supervising post-acute program and medical director(s), creating contractual arrangements, setting and monitoring annual accountabilities, and providing for leadership growth.
  • Demonstrates knowledge of effective quality improvement and change management techniques, and leads the implementation of new models in an effective manner to achieve the organization’s goals.
  • Develops and implements the staffing, education, mentoring, information technology, and data needed to enable post-acute care teams to effectively provide patient- care.
  • Maintains operations by ensuring compliance with all appropriate internal policies and procedures and with external regulatory and accrediting agencies.
  • Ensures policies and procedures are current and support current practices.
  • Develops, prepares, and manages annual approved budget.
  • Participates in leadership growth and development.

Job Requirements

Experience Requirements:   
  • Experience in more than one post-acute care modality: (IRF, LTACH, SNF, Home Health, Hospice).
  • Minimum of 10 years in a director-level role.
  • Minimum of 3 years in an executive level role.
  • Prior experience in care management preferred

Education Requirements:

  • Bachelor’s degree in a clinical discipline or healthcare-related field required
  • Master’s Degree in related field preferred

Licensure or Registration preferred:

  • PT, OT, ST, HFA, or RN
