Job Description

Hours:  Friday, Saturday and Sunday - 8:00 am - 6:30 pm, with back up call responsibilities Saturday and Sunday nights



Major Duties and Responsibilities: 


  • Triages/manages patient/family telephone calls during office hours using clinical knowledge, critical thinking skills, the nursing process, established protocols and evidence based practice principles to identify, analyze and assist with resolving the patient care issues and problems. 

  • Determines if patient/family requires a visit and assigns appropriate on-call nurse to perform patient visit.

  • Documents accurately and timely each patient interaction/call and intervention and other patient/family and physician contacts according to agency policy through use of the electronic medical record.  Documentation should be complete at the end of the day.

  • Communicates and collaborates with the members of the appropriate interdisciplinary team and provider regarding patient needs and care. Contacts patient’s physician as necessary to obtain medical orders.

  • Provides patient and/or caregiver with appropriate instructions to support safe care of the patient in the home.

  • Ensures that patients receive the appropriate level of care within the appropriate timeframe. 

  • Makes follow-up telephone calls to contracted agencies and the patient/family as needed to evaluate care. 

  • Makes home visits as necessary to perform nursing intervention if needed.

  • Completes audits on EMR as directed by the Director of Clinical Services or the PI Coordinator.


Job Requirements

  • Graduate of accredited school of nursing

  • Two years clinical nursing experience

  • Current North Carolina Registered Nurse license

  • Previous hospice work experience preferred.

  • Valid NC driver’s license and reliable independent transportation

  • Ability to maintain positive working relationships and courteous interaction with public

  • Ability to work a flexible schedule

  • Ability to read signs, meters, and hear patient communication
