Location: Newport, NC (part-time - every other Fri, Sat, & Sun 8pm-6am)

ESSENTIAL JOB FUNCTIONS:

1. Creates positive experiences for internal and external customers that will meet their expectations. (External
customers include our patients, families, referral sources, vendors, the community, etc. Internal
customers are the people within the agency with whom you work.) Displays a high degree of courtesy, tact, and
knowledge of services provided by the agency in all contact with staff, patients, and visitors.

2. Administers personal care by following the patient’s individualized plan of care: (a) provides or assists patient with personal care; (b) toileting/elimination; (c) observation reporting and documentation; (d) plans and prepares
nutritious meals satisfying special diet restrictions; (e) safety and universal precautions; (f) provides and/or assists patient with transferring, turning, lifting and walking, remind and encourage physical therapy exercises; (g) assists patient with medications per agency guidelines; (h) ensures and maintains a clean, healthy, tidy and safe
environment; and (i) performs reading, writing tasks, and companionship when necessary; (j) assist with home
management, and errands; (k) builds and maintains rapport with client/family.

3. Immediately reports and records changes in the patient’s mental or physical condition to Home Care Case Manager
or Home Care Manager.

4. Actively participates in the orientation of new staff to patient, family members, and the home setting as requested.

5. Submits documentation as evidenced by reports: (a) utilizes the Mobile Care system daily to very schedules and
care plans; (b) documents tasks performed on the care plan via Mobile Care at the time of the visit; (c) reports to Area Case Manager task consistently not performed and reason; enters employee time via Mobile Care system accurately
and timely; (d) submits accurate time and travel sheets at least weekly; and (e) submits other data to supervisor as
requested on a timely bases.

Job Requirements

-High School Diploma or GED preferred

-Experience preferred

-CPR certification preferred

-Possess valid North Carolina Driver's License

-Must have reliable transportation.
