Albright LIFE consists of three healthcare centers where seniors can spend several days per week receiving the support they need in order to continue living independently at home.  Albright LIFE is part of Albright Care Services, a not-for profit organization which has been serving seniors in Central Pennsylvania for 100 years.

At Albright LIFE Lancaster and LIFE Lebanon, we are looking for a Clinical Intake Coordinator.  Work hours will typically be split equally between the two locations. This is a Monday-to-Friday position with day hours.  Our Clinical Intake Coordinators:

  • Embraces the team and interdisciplinary care concepts in the LIFE Program environment, and educates prospective participants on these concepts. Monitors the prospective participant’s progress toward enrollment; documenting in the medical record in accordance with LIFE procedures; and, working with the LIFE Director of Outreach to monitor key benchmarks for intake and enrollment services.
  • Responsible for the coordination of intake evaluations for Lebanon and Lancaster sites and the promotion of a positive experience for all prospective participants. Work with Enrollment Coordinator(s) as back-up for screening intake calls or walk-in referrals for appropriateness.
  • Responsible for meeting established intake, and census goals; and utilizing Answers on Demand, or other content management system adopted by Albright Care Services, to document necessary information and maintain organized files as required by Albright policy and DHS and CMS regulation.
  • During the intake process, provide to and obtain from families the information necessary to facilitate the intake process. This includes, but is not limited to, the following: Initial Referral Form, Financial Statement, Authorization for Assessment and Release of Information, Medicaid Application, HIPAA notice.
  • Facilitates all intake and assessment steps, including the Intake Home Assessment, family/responsible party meetings, the preadmission visit to the day center if applicable, and meetings with the interdisciplinary team and other staff when appropriate or recommended by the interdisciplinary team or administrative staff.

The starting hourly rate for this position depends on experience but typically ranges from $17.00 to $20.00.

Job Requirements

  • Licensed Practical Nurse preferred, currently licensed in the Commonwealth of Pennsylvania with one year experience as a licensed practical nurse with a frail or elderly population.
  • Ability to interpret existing health regulations, as they affect the PACE (LIFE) model.
  • Ability to effectively plan and coordinate nursing care for individuals, families and groups.
  • Ability to plan, organize, work independently, problem solve under pressure, establish and maintain cooperative working relationships, and effectively supervise and direct the work of others in a fair, firm and consistent manner. Must be able to accept and utilize guidance.
  • Fluency in Spanish and English is preferred.
  • Must possess a valid PA driver’s license and be able to travel and work flexible hours as needed.
