Job Description

Case Manager/Clinician II

Provide case management services to high-risk pregnant and parenting women and children who are affected by substance abuse and/or co-occuring disorders.

Duties to include:

  • Complete assessment, treatment planning, crisis planning as well as clinical intervention and supportive counseling

  • Perform case management duties in a mental health or substance abuse setting

  • Provide clients with care coordination

  • Perform pre-screening, initial interview and service plans

  • Perform case management functions related to service referrals

  • Provide leadership and training

  • Provide clients with enhanced  care coordination with equal focus on both behavioral and primary health

  • Provide hospitalized clients with support with discharge planning

  • Responsible to provide monthly billable services and complete all documents


  • Of service and systems available in the community, including primary health, support services

  • Of different types of assessments

  • Of service planning process

  • Crisis intervention

  • Treatment planning and interviewing techniques

  • Clinical record documentation


  • Strong written and verbal skills

  • Conducting interviews

  • Assessing human problems

  • Utilizing appropriate counseling techniques

  • Ability

  • To work effectively with clients and professionals

  • Ability to communicate

  • Stabilize crisis situations

  • To work independently and as a team

Base Pay: $23.48

Job Requirements


  • Requires Bachelor’s Degree and  experience to seven years in the fields such as Mental Health Counselor, Substance Abuse Counselor or Social Worker

  • Required to meet Virginia Certification requirement for QMHP-A

  • Required to provide VA or NC Driver’s License and DMV Record.

     Base Pay: $23.48
