Job Description

We are seeking Clinician II for Adult Correctional Services

You will be responsible to provide advocacy, monitoring and discharge planning for clients with serious mental illness at the State Hospital and in the Community

Responsibilities to include but not limited to the following:

•   Provide discharge planning and linkage to the community resources

•   Complete intakes, assessments, individual service plans, updates and discharges

•   Conduct face-to-face assessment

•   Preform case management functions

•   Coordinate services and planning with other providers

•   Provide testimony in court

Knowledge

•   Community Services

•   State Licensure Requirement

•   Medicaid/Medicare Billing Requirements

•   Knowledge of Mental Health, Substance Abuse and co-occurring disorders

•   Interviewing & Treatment Planning

•   Case Management Techniques

•   Clinical Record Documentation

•   Conducting Interviews

Job Requirements

Ability

•   Effectively communicate in writing and in person

•   Establish and maintain effective working relationships

•   To follow instruction both written and verbal

•   To document case notes and meet all required deadlines

Special Credentials

•         QMHP-A (Qualified Mental Health Professional for Adults)

Qualifications:

•   Master’s degree in human services or related filed with at least 2 years of mental health experience.

•   Bachelor’s degree with equivalent experience

•   Previous experience with adult criminal justice system and Case Management

•   Experience in conflict resolution

•   Compassionate and caring demeanor

•   Ability to build rapport with clients

•   Excellent written and verbal skills

Employees are required to provide the following at time of hire

•   Certified Transcripts, CPR certification, TB test, VA or NC driver’s license, Car insurance and DMV record.
