Job Description


Direct Support Provider


 Opportunities in the following programs:


 ICF Group Homes


Provide primary direct care and support to adults with mental retardation/intellectual disabilities, physical impairments and medical issues in an intermediate care facility. Ensures that all aspects of the clients’ plan are executed in a timely manner, attend assigned medical appointments & social outings. Schedules are Part Time and include weekends, evenings and overnights.


Part Time Hours-/ Full time Hours Hours will vary- Must be available to work weekends and holidays.


Supportive Living Program-


Provide support, assistance and training in all aspects of lift to adults with a primary diagnosis of intellectual disability, additionally they may have a second secondary diagnosis of mental illness or significant health condition. Support will include but not limited to: skill building an in all activities of daily living skills such as meal planning, personal hygiene, cleaning and maintaining home, laundry, cooking obtaining/taking medications, money management, medical/dental appointment, personal care, transportation and community integration.


Part Time Hours- Hours will vary- Must be available to work weekends and holidays.


 

Adult Day Program


Provide direct care and support in an Adult Day Program. Will participate in the development and implementation of service plans for individuals with intellectual disabilities as well as provide supervision, support and training. Provide instruction to program participants during training activities that encourage independence, assistance with person hygiene, assessing community outings, and other activities of daily living. This position is working in with sever profound (sensory) group.


Part Time Monday – Friday 1:00pm-5:30 pm


 Community Employment Options

Provide supervision and support to four to five intellectually disabled individuals working in a community business including Medicaid Waiver funded clients. Requires the ability to communicate both verbally and in writing/ must be able to work with and relate well to a variety of people in the community including businesses. Will assist at job sites and must be willing to perform manual or physical works as needed. Maybe required to transport clients occasionally.


 Hours will vary depending on program.  Holiday and Weekends

Job Requirements

Qualifications


  • Position requires a combination of education and or experience equivalent to two years with adults who have intellectual disabilities.

  • Proficiency in documentation skills

  • Use of electronic health records required.

  • Proficient in reading, writing, math

  •  Computer skills required.

  • Must be able to lift up to fifty pounds.



Employee to provide the following at time of hire,


  • CPR/First Aid certification,

  • TB test,

  • VA or NC driver’s license

  • DMV Record
