What You Will Do:
As an Occupational Therapist, you will:

  • Evaluate the patient's functional status (muscle function, endurance, visual coordination, written/verbal communication skills, self-care ability, work capacity, etc.), and home environment for hazards or barriers to more independent living; identify equipment needs
  • Identify patient and family/caregiver needs for other home health services and refers as appropriate
  • When the Occupational Therapy is the only skilled service provided, supervised and evaluates Home Health Aide every two (2) weeks
  • Report to the primary physician any changes in the patient's condition
  • Participate in the total plan of care by developing a treatment program, establishing goals for improved functions and communicating the plan of care to the referring physician and other organization professionals
  • Maintain appropriate clinical records and clinical notes, submit these records in accordance with organization policy
  • Maintain contact/communication with other personnel involved in the patient's care to promote coordinated, efficient care
  • Instruct the patient's family/caregiver and other organization health care personnel in the patient's treatment regime when indicated and appropriate
  • Attend and contribute to/in services, case conferences and other meetings as required by organization policy to ensure coordinated and comprehensive plans of care for the patients
  • Supervises Certified Occupational Therapy Assistants according to organization policy and State regulations
  • May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function
  • May train the patient in the use of prosthetic and/or orthotic devices
  • May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury in accordance with Organization policy
  • For patients who plan to return to work, the Occupational Therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Organization policy
  • Supervise Certified Occupational Therapy Assistants according to organization policy and State regulations
  • Participate in discharge planning for the patient
Occupational Therapist
Minimum Qualifications:

  • Registered with the National Registration Examination of the American Occupational Therapy Association
  • Minimum of two (2) years of clinical experience as an occupational therapist
  • Possess a current valid Arizona Class C Driver's license
  • Have reliable personal transportation
  • Have a current CPR and First Aid Certification
  • Have a TB clearance health card that meets federal/state requirements
  • Have a satisfactory fingerprint clearance that meets federal/state requirements
  • Be a US Citizen or have proof of ability to work legally in the United States
  • Ability to successfully pass background checks
  • Bilingual and/or conversational Spanish language ability desired but not required
  • Graduation from an accredited Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education, the American Medical Association, and the American Occupational Therapy Association

Dependable Nurses, Inc. caters to the needs of long-term care (LTC) insurers and their members and our nurses are experts at caring for persons with chronic illness. We accept Mercy Care Plan LTC, Evercare, ALTCS, VA, University Family Care, United Health Care and dual complete, AZ Department of Developmental Disabilities, and private reimbursement.

The client’s chronic needs influence the structure of the authorization for visits and dictates, in a large part, the length of the therapeutic relationship. In contrast to other types of health insurers which focus on short-term periods of service related to episodic illness or injury, LTC insurance providers manage their chronically ill members proactively by sending home health services to provide ongoing skilled nursing assessments aimed at keeping the client from needing expensive hospitalization or extended nursing home stays. LTC insurers employ case managers who maintain long-term relationships with their membership, with care providers such as Dependable Nurses, Inc., as well as with individual nurses.

Effective provision of any care in the home depends on developing the therapeutic relationship. Keeping this in mind, our nurses establish longstanding therapeutic relationships with our clients over months and years. In the event of an acute exacerbation of condition, our aim is to intervene early.
