Job Description

Dependable Health Services is seeking an experienced RN Community Hospice Liaison to JOIN our Dependable Hospice team in Tucson Arizona. The hospice liaison will develop and manage relationships within the medical community to grow and provide hospice to patients in need. As a hospice liaison, the duties are to coordinate hospice admissions based on the criteria set out by Dependable. The hospice liaison reviews patient referral's, collaborates hospice care options with patients and family members, and plan with both the patient and the facility to prepare for hospice care. The hospice liaison will work closely with the hospice marketer, potential hospice candidate , families and their corresponding medical team to ensure the transition to hospice is handled delicately. The hospice liaison communicates with other medical entities and their teams, who may be potential referral sources, and as an advocate for the family or loved one choosing the end of life path through hospice. 
Job Responsibilities:
Coordinate hospice care by working with other team members as well as with the patient/family
Complete hospice admission and evaluate the effectiveness of hospice services to the patient and family ongoing
Nurse liaison will complete admission process that includes securing the physician, any pertinent patient information for hospice admission as well as assisting the patient and family through the process
Once admission process is completed, nurse liaison will initiate and transfer care to Nurse Case Manager
Attend IDG Meetings regularly
Adhere to Federal, state and accreditation requirements including Medicare and Medicaid regulations
Maintain an ongoing knowledge of current drug therapies 
Participate in staff development meetings
Ability to readily network
Marketing plan and collaborate with a team
Referral goals/weekly-informational, presentations, follow-up of past referrals
Excellent organization, planning and skill for hospice presentations
Develop and manage relationships within the medical community  to grow and provide hospice to patients in need
Analyze patient/company relationships
Collaborate with marketing-data
Community liaison communicates to other medical entities and their teams, who may be potential referral resources, and as an advocate for the family or loved one choosing their end of life path through hospice
Provide education to patients/family members regarding the dying process, plan of care and offers guidance with healthcare decision.
Performs on call duties
and other duties as assigned, such as filling in for other nurses

Job Conditions
May be stressful
The ability to access patient's homes which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care
Physical activities will include, walking, sitting, stooping and standing and minimal to maximum lifting of patient's and the turned of patients.
Equipment Operation
Computer, smart phone, thermometer , B/P cuff, glucometer, PT/INR machine, penlight, hand washing materials, PPE, and other various medical and office equipment

Bachelor or Master's preferred Registered Nurse
Must be licensed in the State of Arizona as a Registered Nurse
One year of hospice RN experience
Must be able to communicate verbally and written 
Must be a licensed driver with an insured automobile in good working order
Should be skillful in organization and in the principals of time management and have knowledge of hospice admission and referral process
