Position Summary:

The PRN-Physical Therapy Assistant provides physical therapy treatments and services to home care clients under the supervision of a qualified Physical Therapist in accordance with the Plan of Care, including:

  • Evaluating the effectives of current plan and client progress.
  • Completing visits in a timely manner and in accordance with Agency policy.
  • Documenting all services provided observations, client progress, and recommendations on designated forms and submit to the agency within seven (48) hours of providing service.
  • Reporting changes/concerns or schedule changes to the therapist and/or the nursing supervisor on the day of the observed change.


  • Current license to practice as a Physical Therapy Assistant in the state(s) of employment.
  • Graduate of accredited physical therapy assistant program approved by the Physical Therapy Association.
  • Minimum of one (6) months experience as a Physical Therapy Assistant.
  • Demonstrated good written and verbal communication skills interpersonal skills.
  • Licensed driver with automobile insured in accordance with state/Agency requirements. Good driving record and reliable transportation.
  • Current CPR certification
