ADON/MDS COORDINATOR - RN OR LVN

Brownfield Rehabilitation & Care Center, a 54-bed skilled nursing facility located in Brownfield, Texas assists individuals who may benefit from 24-hour nursing care and assistance with meals, medications or other activities of daily living. At Brownfield Rehabilitation and Care Center, we understand that the need for long-term care, skilled nursing or rehabilitation may not occur under ideal circumstances. It is our goal to make this transition easy and comfortable for all new patients/ residents.

 

Position Summary:

The ADON/MDS Coordinator will work together with the Director of Nursing to ensure quality care and outstanding customer service to all patients and their families, as well as create and maintain great employee relationships among staff. They will also work together with the Director of Nursing in attracting and retaining top preforming, talented team members as well as supervising and career coaching existing team members. The ADON/MDS Coordinator will also be responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care. They will participate in the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM). The ADON/MDS Coordinator will establish and maintain positive relationships with all members of the healthcare team and will function as part of the nursing management team. They are motivated by team building, documentation, and a drive for clinical excellence!

Qualifications:

  • Ideal candidate must be a skilled communicator, director, and motivator
  • Able to organize and prioritize many tasks effectively
  • Must have a current, unencumbered RN/LVN license to practice in the state of -TX; RN preferred
  • 2+ years of experience with MDS in a long term care environment is preferred
  • Must be able to relate information concerning a patient/resident’s condition
  • Ability to follow all applicable policies and procedures as well as current federal, state and local standards, guidelines and regulations that govern long term care facilities
  • Must be able to react to emergency situations appropriately when required
  • Must be able to read, write, speak, and understand the English language
