Job Description

Position Summary:

The Physical Therapist plans and provides physical therapy services to clients in their homes in accordance with the Plan of Care and Agency policies and procedures. The Physical Therapist performs functions that require substantially specialized knowledge, judgment and skill, including:

  • Performing physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.
  • Implementing teaching appropriate to the client’s needs and within the context of his/her environment and learning ability.
  • Assisting in the development, implementation, and evaluation of the ongoing quality improvement process.
  • Promoting personal safety and a safe environment for clients and coworkers.
  • Supervising applicable home care team members.
  • Performing other related duties and responsibilities as deemed necessary.
  • Attending care conferences, initiates interdisciplinary referrals and participates in care coordination as needed.
  • Performing their duties and responsibilities as deemed necessary.

 

Qualifications:

  • Graduate of Physical Therapy program approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association or the Council on Medical Education of the American and the American Physical Therapy Association.
  • Possesses current registration or license as required by the state of practice.
  • Minimum of two (2) year experience in a health care facility or equivalent experience.
  • Demonstrates good written and verbal communication skills interpersonal skills.
  • Licensed driver with automobile insured in accordance with state/Agency requirements. Good driving record and reliable transportation.
  • Current CPR certification.
