For more than 118 years, Backus Hospital has been the primary source of healthcare services for much of Eastern Connecticut. We are home to the only trauma center in New London and Windham counties, and are the only area hospital with LIFE STAR helicopter services. We deliver and coordinate a continuum of high-quality healthcare with the goal of improving the health of our communities.

JOB SUMMARY
In conjunction with the Heart Failure/Cardiology and Pulmonology providers and the Heart & Vascular Institute, the Heart Failure/COPD Program Coordinator is responsible for the development, implementation, coordination and evaluation of the clinical protocols/practice management guidelines related to the care of CHF and COPD patients. The program coordinator will collaborate with all other disciplines to provide education, support and consultation to enhance the quality of care delivered and facilitate optimum development of all services related to Heart Failure and COPD patients.

Job Requirements

Education: Bachelor’s Degree in nursing required, additional Master's Degree in health related or business field preferred.


Experience: At least three (3) years of experience in a clinical area (such as cardiology) or program management; three (3) years of cardiology/heart failure experience preferred.

Licensure, Certification, Registration: Current Connecticut License as Registered Nurse Required. Heart Failure certification preferred.

Language Skills: Highly effective verbal and written communication skills.

Knowledge, Skills and Ability Requirements
• Ability to work effectively with providers and other departments
• Computer literate and database managing experience
