Our Primary Care Nurses (RN) utilize their extensive education and training to provide personalized care to our patients in a rewarding environment that promotes autonomy. 

1.         Prepares and maintains all Hospice project documentation including initiation, delivery plans, budgets, system testing, deployment planning and training business associates as needed.

2.         Ensures projects are executed in line with proper project management protocol, including building project plans, identifying project scope and objective, securing project resources, funds and identifying alliances.  Assigns project tasks and timeline deliverables.

3.         Develops and maintains project communication documentation for team members and project stakeholders and establishes a disciplined and regularly scheduled distribution and reporting process.  .

4.         Coordinates internal business meetings with appropriate parties to discuss project initiatives, request support and as appropriate, proactively identify and undertake activities to discuss and address problems inhibiting progress.

5.         Delegates tasks, responsibilities and authorities effectively.  Identifies goals and objectives and motivates and leads others towards their achievement.  Is able to network across business units and within the community as project initiative requires.

6.         Identifies project risks and inter-dependencies and where possible plans for contingencies and allowance of potential problems.

7.         Anticipates and understands customer expectations and ensures customer requirements are met and expectations are appropriately communicated and managed.

Job Requirements


Bachelor’s degree in business administration or other appropriate discipline. 


1.         Minimum of three years project management experience in a role that requires involvement and understanding of project management initiatives including introduction of new business processes to support business goals.

2.         Experience leading multi-functional projects that require multidisciplinary alignment and coordination.

3.         Three years’ experience in Homecare data analytics with in-depth excel capabilities.

4.         Experienced in change management, trained in project management disciplines and three years Epic experience.

5.         Minimum of three years Homecare experience
