Job Description

CORE COMPETENCIES Reporting to the Director, the Social Workers primary responsibility is to initiate, plan and implement programs designed to identify and meet social and emotional needs of residents and their families.

• Ensures that the highest degree of quality care is delivered and maintained at all times

• Responsible for providing support in services related to the physical, psychological, and social needs of the residents.

• Works with families to assist them in coping with residential care, illness, diagnosis, and/or life situation and to meet the continuing needs of residents

• Coordinate interdisciplinary team to provide person-centered care for each resident/guest and their caregiver/family through all phases of their experience, pre-admission, stay/period of service, and discharge

• Works closely and collaboratively with the clinical team to identify appropriate admissions; ensure that resident care objectives are established and met; that the standards, policies and procedures are consistent with the standards, policies and procedures of NLCS, and current, state and federal standards of care and practice as required

• Provides psychosocial assessments of residents to identify any emotional, social, and environmental strengths and concerns; participates in preparing and maintaining social history, care plan and MDS for each resident

• Manages scheduling of resident move-in process with Environmental Services, Nursing, Admissions and other related departments. Coordinates with residents and families regarding move-in process. Assists residents and families in processing fears, complaints and frustrations. Sets up family meetings as needed. Provides potential psychological impact notification to facility interdisciplinary team as necessitated

• Assists residents and families at admission and in the post admission adjustment period.

• Coordinates and hosts resident care planning meetings to include invitation to residents and families.

• Coordinates discharge plans for residents with internal staff and outside vendors.

• Educate residents and families about community resources.

• Performs other duties as assigned JOB QUALIFICATIONS

• Bachelors degree in Social Work required, Masters degree in Social Work preferred

• Licensed Social Worker by the state of assignment (Maryland)

• Five to ten years experience required preferably in a long-term care or senior health care environment

• Visibility requires maintaining a professional appearance and providing a positive company image

• Must possess strong written and oral communication skills and ability to maintain confidentiality of information

• Computer electronic medical record experience is required

• Demonstrated ability to work effectively with a diverse group of stakeholders

• Must be proficient in Microsoft Office
