Job Description

Summary: The Behavioral Health Consultant (BHC) requires an independent license to practice in a health care setting such as an LCSW (licensed clinical social worker) or LCPC (licensed clinical professional counselor). The BHC is a regular team member at primary care clinics and delivers brief, consultation-based services to patients and primary care providers (PCP) as well as conducting groups. The BHC works with individuals, groups and families, providing evidence-based behavioral interventions for both mental and physical health conditions in patients of all ages. The individual in this position must be comfortable with a flexible schedule, able to complete at least 10 patient visits per day, and adept in working closely with PCP’s and other team members. Other required skills and duties are outlined below.

• Screen primary care patients through warm hand-offs as requested by the primary care providers/clinical staff, and develop a plan of action to address presenting needs. Work as part of an interdisciplinary team, communicate effectively with primary care providers/clinical staff to provide patient care and coordination.

• Provide brief targeted interventions usually lasting 30 minutes or less and short term counseling if needed often lasting less than 8 visits. Develop and conduct groups. Propose and implement services that improve the health of patients and assist with the implementation of the primary care provider’s treatment plan.

• Manage the psychosocial aspects of chronic and acute diseases, application of behavioral principles to address lifestyle and health risk issues, consultation and co management in the treatment of mental disorders and psychosocial issues.

• Provide primary care providers timely feedback about patient’s care, treatment recommendations and progress via documentation in the patients’ records and verbal feedback.

• Ability to work effectively with a diverse patient and staff population and use of interpreter services as needed.

• Basic understanding of primary care and nontechnical knowledge of psychotropic and other relevant medications.

• Assist with care coordination and access to outside resources for patients as needed. Follow up and monitor completion and outcomes of referral for at risk patients. Develops and maintains relationships with external providers, case managers and hospitals.

• Attends, participates in and reports on various internal and external meetings and trainings.

• Participates in evaluation of peers and support staff.

• Excellent computer skills, able to work in an electronic record. Maintains documentation of patient’s individual and group treatment by using current and accurate ICD-Coding and CPT coding.

• Establishes and maintains a professional and therapeutic relationship within the standards of patient profiles, confidentiality laws and the newest clinical research available.

• Attend trainings, workshops and other relevant professional development in order to maintain licensure.

• Adheres to professional standards as outlined by governmental bodies, NASW (and/or other appropriate professional associations), private funding sources, organization plans/policies and organization’s guidelines. Participates in periodic evaluative reviews and/or in house and external staff trainings.

Education and/or Experience:

• Master’s degree in clinical social work, counseling or clinical psychology; and three years of clinical practice experience in either a hospital or community setting providing group and/or individual therapy, required.

• Licensure as an LCSW or LCPC required.

Language Skills:

• Bilingual English/Spanish proficiency required.

• Excellent written and oral communication skills required.

• Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

• Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Competencies: Ability to use computers to receive, organize, prioritize, and transmit information. Proficiency in use of Microsoft Office suite including Outlook, Word, and Excel. Comprehension of and appropriate use of basic medical terminology, application of third party payer guidelines and concepts related to quality assurance measures. Compliance with organization policies and procedures.
