Job Description

Summary: The Integrated Care Nurse will work closely with the behavioral health team, which includes psychiatrists, psychiatric nurse practitioners and social workers. The Integrated Care Nurse will act as a liaison between the primary care providers and the behavioral health staff to ensure integration of patient care as well as good communication between providers. The Integrated Care Nurse will work with the rest of the medical team to assist with direction of daily patient visits, providing assessment and triage, care coordination, and health education and information to patients and families in a culturally appropriate and relevant manner. The Integrated Care Nurse will work with the health center team to assure the maintenance of a safe patient care environment and safe transitions in care both within HHC and with other community agencies, and contribute to quality assurance and other organizational activities as needed.   

Essential Duties and Responsibilities:

  • Triages walk-in visits and telephone calls to the nurse line, facilitates medication refills, and administers medications.
  • Provides oversight in the coordination of referrals, including accessing necessary values and reports, providing information and support to patients and families, and working with staff to assure appropriate follow up. 
  • Participates in weekly integrated care meetings
  • Provides health information, promotes health and well-being for patients and families by assessing health literacy, providing meaningful health information, and supporting self-management of chronic physical and mental illness
  • Supports Medical Assistants by providing information, ongoing development and training.
  • Performs clinical patient visits that may include: administering immunizations and medications, providing education or follow-up on diabetes care, asthma or other chronic disease management, administering STI treatment, Emergency contraception, Depo Provera injections, PPD placement/reading
  • Actively identifies community referrals, fosters relationships with community agencies, other community health activities
  • Additional Responsibilities (based on the clinic site and care team):

       -Serve as “Super User” on Electronic Medical Record (EMR)

       -Provide group health education

  • Education and/or Experience:  Active Illinois Registered Nurse License and CPR certification required.  Must have at least 1 year of behavioral health experience. Preferred BSN or Master’s degree.
  • Communication Skills: Excellent verbal and written communication skills
  • Language Skills: Bilingual in Spanish/English preferred
  • Analytical Skills:  Ability to read and interpret documents
  • Mathematical Skills: Ability to add and subtract two digit numbers and multiply and divide with 10’s and 100’s.
  • Problem Solving: Ability to define problems, collect data, establish facts, and draw valid conclusions
  • Travel: Minimal, occasional local travel for meetings between clinic sites
