Job Description

Hours:  Saturday and Sunday 7:00 am - 7:30 pm and Monday 7:00 am - 3:30 pm

  • Assesses physical, environmental/safety needs of patient.

  • Develops and coordinates plan of care in collaboration with patient, significant others, and interdisciplinary care team members to focus on holistic needs of patient in order to assure satisfactory outcomes for patient.

  • Provides in-patient facility nursing care according to plan of care.

  • Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.

  • Accurately and timely documents nursing assessment, interventions, and updates to Plan of Care according to the nursing standards of this organization. 

  • Supervises nurse aides and documents supervision according to organizational policy. 

  • Provides caregiver with appropriate instruction to support safe care of the patient at Hospice Home at High Point and in accordance with the current plan of care.

  • Complies with organizational nursing standards.

  • Maintains patient confidentiality at all times

Job Requirements


  • Graduate of accredited school of nursing – Associates Degree, (BSN preferred)

  • Two years clinical nursing experience

  • Current North Carolina Registered Nurse license

  • Ability to maintain positive working relationships and courteous interaction with public

  • Ability to read signs, meters, and hear patient communication

Physical Factors

  • Unpredictable and varying situations in the in-patient environment

  • Ability to transfer patients and carry DME

  • Risk of exposure to blood or body fluids, infectious diseases, sharp objects, chemotherapy agents, and trauma secondary to automobile accident. 

  • Prolonged standing, walking and sitting

  • Prolonged computer exposure
