Hospice On-Call RN Field Case Manager (FT Position Available!)



The ideal candidate would manage and performs direct patient care within the scope of the Registered Nurse Practice Act based on the nursing process. Plan, coordinate, oversee and execute an individualized, holistic plan of care of assigned patients.


  • Perform direct patient care based on the nursing process and within the scope of the Registered Nurse Practice Act, including identifying nursing problems, psychosocial, spiritual and emotional issues as they relate to the individual needs of the patient and family.
  • Performs Admissions and On-Call duties as necessary.
  • Assist with pain and symptom control related to patients.
  • Assist patient/family with acceptance, cooperation and response to the hospice concept.
  • Prepare a weekly visit schedule and submit to PCM.
  • Report any significant changes in patient’s condition to the attending Physician and/or Patient Care Manager and request change orders as needed.
  • Instruct clients on appropriate techniques for care of the patient and documents following established Plan of Care..
  • Coordinate, assign and supervise LVN and Home Health Aide activities.
  • Participate in training programs for community and area activities related to hospice philosophy including speaking to outside organizations as needed.
  • Responsible for timely completion of paperwork required to meet State/Federal regulations. Documentation should be accurate and detailed to meet these regulations.
  • Participate in orientation of staff and continued education of medical students, nursing students, and other health care professionals including community as appropriate.
  • Maintain required CEU credits according to established standards.
  • Understands and adheres to organizational compliance and privacy programs.
  • Assists in providing service that fosters the organization’s mission, values, vision and goals.
  • Performs other duties as required and/or requested by management.
