Pedi/Adult Hospice Registered Nurse (RN) Case Manager - FT position available!

Summary:

The Pedi RN Field Case Manager is a registered nurse and an Interdisciplinary Team Member whose primary responsibility is to provide professional care with a personal touch. This is accomplished via Houston Hospice.


Ideal candidate would perform initial assessment, admissions, manage and perform direct patient care within the scope of the Registered Nurse Practice Act based on the nursing process.  Plan, coordinate, oversee and execute an individualized, holistic plan of care of assigned patients on Pediatric Team. 
  
Duties/Responsibilities:

  • Perform direct patient care based on the nursing process and within the scope of the Registered Nurse Practice Act, including identifying nursing problems, psychosocial, spiritual and emotional issues as they relate to the individual needs of the patient and family
  • Assist with pain and symptom control related to patients
  • Assist patient/family with acceptance, cooperation and response to the hospice concept
  • Prepare a weekly visit schedule and submit to PCM
  • Report any significant changes in patient’s condition to the attending Physician and/or Patient Care Manager and request change orders as needed
  • Instruct clients on appropriate techniques for care of the patient and documents following established Plan of Care
  • Understands and adheres to organizational compliance and privacy programs
  • Assists in providing service that fosters the organization’s mission, values, vision and goals
  • Performs other duties as required and/or requested by management

Job Requirements


Pedi/Adult Hospice Registered Nurse (RN) Case Manager - FT position available!

  • Graduate of an accredited school of nursing with current Texas license
  • Bachelor’s degree in nursing with home health, hospice, or public health nursing experience preferred
  • Currently licensed as an RN in the State of Texas
  • Current certification in basic life support
  • Excellent administrative skills with ability to supervise staff and coordinate services
  • Excellent communication skills, both oral and written
  • Excellent interpersonal skills
  • Ability to accommodate multi-faith beliefs and backgrounds of patients/families
  • Ability to function as part of interdisciplinary team
  • Ability to travel to multi-work locations during workday
  • Knowledge of applicable State/Federal laws governing hospice licensure
  • Must have reliable vehicle, valid Texas Driver’s License and Liability insurance
  • PC experience preferred
