Medix has an opening for a Bilingual Registered Nurse!


The Registered Nurse for Care Management will act as a liaison for members, managers and facilities throughout the continuum of of care. This is a great way to get your foot in the door with one of the premiere health organizations in Chicago!


We are looking at getting candidates started for this Registered Nurse position immediately!

Summary:

Our Case Manager will collaborate with the CM team, assist with high risk patients who are transitioning to the hospital into a medical home. Educating patients on preventive care, self management, medication management. Telephonic outreach to high risk patients when necessary. The population is primarily medicaid. They will be working with mostly adults, but may have some kids in their case load.

PREFERRED EDUCATION, EXPERIENCE AND SKILLS


  • 1-2 years of Nursing Experience
  • Bilingual in Spanish
  • Experience in case management
  • Must have RN license in IL
