After Hours Home Health Nurse

On Call, FTE: 0; When scheduled, hours of work will be 5:00 p.m. to 8:00 a.m. Also needs to be available for weekend/holidays rotation.

Job Summary:

  1. The After Hours Nurse has the responsibility, accountability, and the authority to deliver safe, effective, quality, excellent nursing care according to the philosophy and goals/objectives of the Department of Nursing.  
  2. Utilizing the Jean Watson’s Theory of Caring, the Relationship Based Care, and the NorthBay’s professional nursing practice model assesses, plans, implements and evaluates care for all patients assigned to his/her care. 
  3. Responsible for responding to after-hours phone calls initiated by the
    physician and/or patient/caregiver, Intake Department, Managers, Case Managers, and providing or coordinating the appropriate level of care for home care and hospice during hours other than regular working hours of the agency. 
  4. Care is provided according to the specific physiologic, developmental and psycho-social needs of the patient. 
  5. Supports the mission, vision, values and expectation of excellence of the organization. 
                            

Education: 

  1. Graduate from accredited school of nursing. BSN preferred.
  2. Minimal nursing degree of BSN level required; current incumbents must be compliant by 2020 unless otherwise meeting defined exemption criteria.  

Licensure/Certification: 

  1. Valid, unrestricted and current RN license by the State of California.
  2. Current AHA or AHA equivalent BLS.
  3. Oasis certification or certification within one year of employment.

Experience, Knowledge and Skills: 

  1. One year of work related experience as a registered nurse in home care, or 2+ years in medical-surgical nursing required. 
  2. Hospice or experience with terminally ill patients preferred.
  3. Proficient written, verbal, and electronic communication skills in the English language.
  4. Good assessment and patient care skills. 
  5.  IV skills, experience with pain management and symptom control preferred. 
  6. Demonstrated sound clinical judgment.
  7. Must demonstrate and maintain current knowledge and skill in providing appropriate care for patients in the following age groups: Geriatric and Adult.
  8. Effective human relations skills, capable of communicating with others in a kind, courteous, and professional manner.  
  9. Ability to work well as a team member. 
  10. Current, valid and unrestricted California driver’s license   
  11. Access to operating automobile with adequate insurance coverage at or exceeding the limits set by the Risk Management Department.
