Licensed Clinical Social Worker - Center for Primary Care

Full Time, FTE: 1; Monday through Friday, clinic hours
FF Center for Primary Care
Fairfield, CA

In this role you will:

  1. Provide direct medical social work, counseling and case management services to patients and their families including triage, crisis intervention, psycho-social assessment, development of a plan of care, implementation and documentation of emotional, social, financial, environmental or follow-up referral needs. 
  2. Develop, coordinate, and conduct pre-screening protocol for primary care patients with co-occurring illnesses.  
  3. Assess patients, develops a psycho-social plan of care within established timeline and monitor progress toward goals. 
  4. Facilitate integration with community-based resources to optimize patient health including outreach to and collaboration with community partners.
  5. Work collaboratively with interdisciplinary members and other colleagues within the NorthBay Healthcare system. 
  6. Regularly attend provider meetings, case conferences, and planning and development meetings. 
  7. Provides on call and acute care to patients; working closely with medical care providers to address presenting issues.


For the position you will need:


  1. Master of Social Work from a school of Social Work accredited by the Council on Social Work Education required. 


  1. California BBS Licensed Clinical Social Worker (LCSW).


  1. Minimum of two year’s experience within the acute or primary health care setting or the community health setting. 
  2. Experience with mental disorders and medical comorbidity preferred.


  1. Strong psycho-social assessment skills.
  2. Strong knowledge of medical diagnoses and concerns.
  3. Strong critical thinking skills.
  4. Skilled at working with patients and families coping with co-occurring illnesses and/or multiple stressors.
  5. Excellent follow through skills.
  6. Strong organizational skills with attention to detail required.
  7. Must be capable of sound and independent judgment and decision-making.
  8. Must have working knowledge of motivational theories, system theories, developmental theories, and group dynamics. 
  9. Ability to work effectively with diverse populations, including those with respect to age, culture, ethnicity, and gender.
  10. Must have strong written and oral communication skills.
  11. Basic proficiency with word processing required; basic proficiency with databases strongly preferred.   
  12. Bi-lingual Spanish strongly preferred.
