Primary Care Team RN
Full Time, FTE:  1; Monday - Friday 8:00am - 5:00pm

Job Summary:

The role of the Outpatient Primary Care Team Registered Nurse (RN) is to function as a member of the multidisciplinary team. The Team RN utilizes clinical knowledge and competence, positive communication skills, problem solving and conflict resolution techniques, ability to effect change, strong skills in assessment, organization and time management. The Team RN will be familiar with the case management process requiring a focus on customer service skills, knowledge of setting appropriate goals and measuring outcomes to effectively ensure optimal client outcomes with consideration to payer requirements.


  1. Graduated from an accredited nursing school or college.
  2. BSN required.

 License and Certifications:

  1. Current California state RN license required.
  2. AHA or equivalent BLS.


  1. One year inpatient or equivalent nursing experience required. Relevant outpatient clinical experience preferred. 
  2. Relevant outpatient clinical experience preferred.
  3. Minimum of one year case management experience preferred; equivalent case management education will be considered.

Knowledge and Skills:

  1. One year inpatient or equivalent nursing experience required.
  2. Must have knowledge of:  Standards of federal and state regulations; Diagnosis codes and related guidelines; Current standards and trends in health care, best practices, and familiarity with related resources and literature; and Capitiation and Managed Care legislation. 
  3. Computing skills including experience with MS Office (Excel, Word, PowerPoint, etc.) and data management are required. 
  4. Ability to effectively communicate with all levels of patients, physicians, health care personnel, supervisory staff, and peers. 
  5. Demonstrates the exceptional ability to be organized and efficient in prioritizing and managing assignments with minimal oversight and direction. 
  6. Demonstrates the willingness to research, learn, and obtain knowledge for the performance of the position. 
  7. Demonstrates a courteous professional demeanor at all times while working collaboratively or individually.
  8. Maintains positive relationships with all team members including peers and supervisors. 
  9. Utilizes critical thinking, applies sound clinical judgment and uses assessment skills for decision making.
