We are recruiting for a part-time Licensed Practical Nurse for our group home in Thomaston, CT. The hours are Thur 11p-7a, Sat 3-11p and Sun 3-11p. 

The LPN works under the general supervision of the Program Manager under the direction of the Group Home Nurse Coordinator with regard to health/medical related issues.  This individual has no supervisory responsibility.

The LPN on the job will have contact with residents/program participants, other employees, medical personnel, parents, guardians and others in the community.


  • Becomes familiar with, and implements Oak Hill's policies and procedures designed to fulfill specified goals and objectives of programs for residents/program participants by providing assistance, guidance and instruction in activities of daily living;  personal hygiene; meal preparation, the serving and feeding of meals; housekeeping activities; social and recreational activities; behavior modification; medication administration and other activities related to maintaining residents'/program participants’ well-being.
  • Attends to the health needs of residents/program participants by becoming familiar with and maintaining current knowledge of their health issues: carrying out instructions pertaining to medical concerns as given by the physician, Group Home Nurse, (GHNC) Manager or Program Supervisor.
  • Maintains resident/program participant health by contacting the physician in medical emergencies such as 911, writing an incident report; and keeping the Manager or Program Supervisor and Group Home Nurse informed.
  • Facilitates resident/program participant care and provides awareness and access to important health related information by taking doctors' orders and recording the information on medication Kardex, medication cards, nursing notes, and in the shift to shift communication book.

Job Requirements


  • Experience, and/or training which provides the knowledge, abilities and skills listed below.  State of Connecticut Practical Nursing License.

Knowledge, Ability, and Skills

  • Knowledge and ability to assess, diagnose and follow treatment procedures for medical problems in normal or emergency situations:  The ability to monitor vital signs, osculation, palpation, and percussion:  Knowledge and ability to manage G. Tubes:  Knowledge of issues surrounding the use of medications and the ability to recognize and respond correctly to such issues
  • Knowledge of the needs of individuals with disabilities; knowledge and ability to maintain a safe environment;  ability to train residents in self-care habits, personal hygiene and proper social conduct.
