Job Description

Psychotherapeutic Services is seeking a Registered Nurse (FT) for our Assertive Community Integration & Support Team (ACIST).

Our ACIST team provides ongoing outreach, monitoring, behavioral health treatment, medication management, case management, crisis intervention, clinic services, and monitoring of medical conditions through service coordination and behavior analysis. 

Our team supports individuals with ID/DD/Autism and severe and persistent mental illness using an intensive case management model unique in the ID/DD/AUTISM community and provides trauma informed, recovery-oriented services in a person-centered manner, with the goal of improving persons' served quality of life by increasing their support system with full community inclusion.

*Arranges for and monitors medical/dental/vision needs of persons served;

* Completes admission requirements and admission orientation of health & medical services;

* Responsible for coordinating psychiatric/medical appointments with physicians

*Ensures evaluations are filed and professional staff are updated routinely;

*Responsible for health assessments and completion/updates, and ensure staff are routinely informed;

*Monitors follow-up of doctor-ordered treatment and laboratory tests;

*Updates family/guardians as needed regarding person’s health status;

*Attends treatment meetings and provides input regarding medical needs as requested;

*Reviews and attends to any medically complex issues when indicated.

Job Requirements

*RN licensed by the Delaware Board of Nursing;

*Experience providing services to persons with ID/DD/Autism and severe and persistent mental illness.
