Community Nurse Supervisor, Covering mainly Westchester county and some areas in the Bronx, NY
Position Summary
The Community Nurse Supervisor is responsible for the supervision of Nursing and Personal Care services provided to Community Clients. This includes the planning, coordination and management of all ongoing Community Clinical Services. This position supervises Community staff to ensure that all agency policies are followed in accordance with all NYS regulatory mandates for home care services. As a representative of Home Care, and of the organization, it is required that the Community Nurse Supervisor perform the essential functions of the job in a professional manner, with a positive attitude and is aware and supportive of the mission, vision, resident Bill of Rights, policies and procedures of the organization.
Essential Job Functions
  • Performs initial assessments of newly referred patients to assess their care needs and determine their eligibility for LHCSA services.              
  • Communicates pertinent clinical information to V P on a daily basis.
  • Develops and implements treatment plans in conjunction with physician orders.     
  • Performs ongoing patient assessments and modifies treatment plan accordingly in coordination with physician, patient and family/significant others.       
  • Provides skilled nursing services to home care patients based on physician orders and following Home Care Policies and Procedures.           
  • Provides ongoing teaching to patient and family as needed to support health and wellness outcomes.       
  • Coordinates all health-related services for all Community Clients.    
  • Oversees all disciplines, including skilled and paraprofessional staff, in carrying out the plan of treatment.       
  • Reviews reports from involved disciplines; reevaluates needs and plans as indicated.            
  • Conferences with coordinators regarding the placement of the 'right” HHA for individual clients.     
  • Facilitates and coordinates referrals to other services in the Campus as needed to meet the patient's needs to be safe and well at home.
  • Participates in all Performance Improvement initiatives.       
  • Ensures all clinical documentation is in compliance with regulatory compliance.
  • Supervises and evaluates all professional and paraprofessional staff in carrying out the agreed-upon treatment plan.
  • Interprets and reinforces policies and procedures with professional and paraprofessional staff.
  • Participates in the planning and execution of the staff education.    
  • Performs annual field evaluations of professional and paraprofessional staff (in home/and annual YE evaluation)
  • Manages staff and oversees care provided to contract cases.
  • Ensures that all clinical documentation is in compliance with third party mandates.
  • May partner with Community Liaison in some Community outreach.              
  • Liaison to other providers of care and services to seniors in the community.
  • Must understand processes and be able to respond in all emergency situations as per company policies.       
  • Attend all mandatory and necessary training offered in order to stay up to date with policies, procedures, etc. 
Knowledge and Skills
  • Must possess a current NYS Licensure to practice as a RN.
  • Must be CPR Certified.
  • Bachelor of Science Nursing preferred.
  • 3-5 years of experience in home care or acute care setting.
  • Must possess the ability to work with others in the organization.
  • Must display patience, tact, enthusiasm and a cheerful deposition.
  • Must have knowledge of basic nursing care techniques.
  • Must have computer literacy and ability to train effectively on new patient care systems.
  • Comprehensive knowledge of nursing service policies and procedures.
  • Must be able to relate information concerning a resident's condition.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must be willing to learn new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must possess strong leadership skills.
