Hours: Full-time, 40 hours/week, M-F, 8-5

Comprehensive Care Coordination

Job Summary/Position Objective:
Assist in activities of care coordination, case management and disease management for Salem Clinic patients.

Essential Functions: 

  1. Work with RNs and physicians to identify and standardize best-practice, evidence-based solutions for patient transitions in care.

  2. Work in collaboration with RNs and physicians to follow up on patient test results, consult reports, and care plan outcomes, and communicate adjustments in plan of care to appropriate parties.

  3. Ensure there is communication with patients and family as appropriate to maximize care opportunities.

  4. Facilitate patient access to services, educational programs and understanding of medical issues.

  5. Work with RNs to develop strategies and interventions to improve compliance with plan of care among patients.

  6. Promote positive team dynamics within department and inter-departmentally.

  7. Alert Director to any potential practice liabilities.

Other Duties:

  1. Participate in professional development activities and professional affiliations. Attend periodic conferences and workshops, some of which are leadership related.

  2. Other duties assigned.

Job Requirements

Required Knowledge, Skills, Abilities: 

  1. Possession of an Oregon State LPN license.
  2. Two years experience working in a medical facility.
  3. Effective customer service skills and a positive attitude.
  4. Strong written, verbal and telephone communication skills.
  5. Excellent organizational skills with the ability to pay close attention to detail.
  6. Ability to work in a high volume environment.
  7. Ability to exercise initiative and sound judgement.
  8. Ability to establish effective working relationships with staff members.
  9. Ability to use a computer. Proficient in keyboarding skills.
  10. Knowledge principles and skills needed to provide nursing assessment, intervention and treatment.
  11. Knowledge of medications and their side effects on patients.
  12. Knowledge of patient education principles to assist patients in caring for themselves.

Preferred Qualifications: 

  1. Experience and/or training in the following areas: Medical Home processes; health promotion; symptom triage and health decision support programs; counseling; patient education and self-management goal setting.
  2. Experience and/or training in population health management and managed care.
