Job Summary/Position Objective:  Assist in activities of care coordination, care management, disease management, and managed care for Salem Clinic patients. 

Essential Functions:

Comprehensive Care Coordination
1.    Coordinate individualized care plans for patients during transitions of care, patients identified as high risk, or patients with high ED utilization.
2.    Facilitate patient access to services, educational programs, and understanding of their medical issues.
3.    Assist to develop strategies and interventions to improve patient’s compliance and understanding with plan of care.
4.    Provide medication and allergy reconciliation.
5.    Promote positive team dynamics within department and interdepartmentally.
6.    Alert Director to any potential practice liabilities.  

Managed  Care
1.    Coordinate inpatient review and discharge planning.
2.    Review DME, surgery, diagnostic, specialty referrals, home health and skilled nursing requests for medical necessity.
3.    Attend Utilization Review Committee meetings as needed.
4.    Prepare denial notices to patients as need according to plan requirements.

Job Requirements

Required Knowledge, Skills, Abilities: 
1.    Possession of an Oregon State RN license.
2.    Two years experience working in a medical facility.
3.    Effective customer service skills and a positive attitude.
4.    Strong written, verbal and telephone communication skills.
5.    Excellent organizational skills with the ability to pay close attention to detail.
6.    Ability to work in a high volume environment.
7.    Ability to exercise initiative and sound judgment.
8.    Ability to establish effective working relationships with staff and physicians.
9.    Ability to use a computer.  Proficient keyboarding skills.
10.    Knowledge of principles and skills needed to provide nursing assessment, intervention, and treatment.   
11.    Knowledge of medications and their effects on patients. 
12.    Knowledge of patient education principles to assist patients in caring for themselves.
