Job Description

CLASS TITLE: CASE MANAGER (CM)

CLASSIFICATION: One FTE- Exempt (hourly)

DUTIES AND RESPONSIBILITIES OF THE CASE MANAGER:

1.   Follow up with Admissions Coordinator to ensure consent forms are signed and update signatures to maintain compliance.

  1. Orientate the clients to SCOA expectations and complete intake packet.
  2. Make contact with teams within 24 hours of client intake.
  3. Update client info in database on intake and throughout client’s stay at SCOA.
  4. Keep accurate documentation of Agency/Family interactions and maintain in chart and in database. (Including satisfaction surveys).
  5. Enroll clients in school and attend IEP and any other school meetings, if applicable.
  6. Provide and/or arrange transportation to current clients for court, home passes, etc, as needed.
  7. Schedule needed physicals, TB’s, dental, and medical appointments, as needed and communicate with team as well as coordinating transportation.
  8. Appear in court as needed, telephonic or in person.
  9. Relay information weekly to teams to maintain good communication between SCOA, agencies, and families.
  10. Act as a liaison for SCOA and outside agencies including schools and community.
  11. Assist Marketing/Admissions Department when needed.
  12. Responsible for clinical charts (correct order, needed paperwork, audits of each chart, once per month and at least 2 RAC’s weekly, RAC updates and following up on outstanding items, etc.).
  13. Schedule, coordinate and facilitate in CFT/AFT meetings and facilitate clients staffing (service plans, signature pages, and notes).
  14. Compile 30 day reports for each client and review for accuracy and completeness.
  15. Send information as requested by current team members in accordance with ROI.
  16. Assist clients in preparing for discharge in conjunction with agency guidelines.
  17. Check voicemail and emails daily and follow up within one business day.
  18. Notify teams of incidents during normal business hours.
  19. Obtain written guardian consent for out of county trips, haircuts, piercings, etc.
  20. Request and distribute DES and other funds as appropriate.
  21. Communicate discharges externally with teams and internally with finance, psychiatrist, admissions, directors’ teams and RN.
  22. Participate in required meetings and trainings including GENOPS.
  23. Ensures all authorizations are completed, current and in file as well as communicated to appropriate departments.
  24. Maintain compliance with HR file.
  25. Any other duties assigned by the supervisor.
  26. Adheres to Sequel policy and practice concerning creation, editing, transmission, storage, and disposition of all Protected Health Information in compliance with HIPAA.
