Job Description

The mission of SequelCare of Oklahoma is to improve the quality of life for persons in southeast Oklahoma by providing comprehensive behavioral, emotional and substance use services designed to enhance and enrich the lives of children, youth, adults and families.  SequelCare of Oklahoma is a nationally accredited (CARF) behavioral health treatment center.

Position Overview:  Aids individuals and families having problems concerning family relationships or other aspects of their social functioning affecting unity of family and welfare of community by performing the following duties.

Primary Responsibilities: 

  • Mediate between the consumer / family and professionals
  • Provide Support / Advocacy for consumer and families
  • Complete SOC forms for every interaction with consumer / family
  • Be a member of the Family Team to empower families to meet their goals and objectives

Education/Experience:

Associate's degree (A. A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
