Sequel Youth Services of Mt Pleasant, Utah seeks a LPN.

Mt Pleasant Academy helps each struggling teen in our care find their inherent worth and value through experiential based experiences. We likewise employ individuals dedicated to positively influencing these adolescents in working through histories of poor self-esteem, poor coping skills, lack of anger management skills, anger, depression, self-harm, drug/alcohol abuse, sexual abuse, poor school performance, abuse/neglect, etc.

Job Duties Include

  • Responsible for oversight of individual unit.
  • Conduct initial medical assessment and document identified concerns.
  • Complete assessment and progress notes within the required guidelines of the hospital and regulatory agencies.
  • Document patient progress and changes via assessment notes and progress notes.
  • Perform risk/lethality evaluations on “at risk” patients within the scope of training and licensure.
  • Ensure electronic patient files are current and in compliance according to the policies and procedures.
  • Monitors and evaluates patients in the context of individual and group treatment for progress or the development of additional symptoms of mental/behavioral disorders.
  • Perform medication inventory and practices safe administration of medications.
  • Obtains and documents physician’s authorization regarding medication changes.
  • Reports laboratory reports to the physician.

Position Requirements

  • Diploma from a state-approved training institution.
  • Current Utah LPN license.
  • Minimum of one year’s experience in the behavioral health field preferred.

Sequel Youth Services offers a full benefits package for full time employees, including health, dental, vision, supplementary insurances and 401(k) after 60 days of employment, Daily Pay after 30 days of employment.
