Program Overview:

Sequel Youth and Family Services of Owens Cross Roads, is an intensive residential treatment facility providing an array of gender specific, evidenced based and trauma informed services for females, ages 12-1eight, with serious and/ or chronic, emotional and/ or behavioral management problems that interfere with the child's ability to function in the family, school, and/ or community setting outside of a residential environment. We have an immediate need for a full-time Masters Level Case Manager.

Position Summary:

The Case Manager coordinates and delivers various programmatic interventions based upon strengths and needs identified in a treatment or service plan.  Interventions include individual, group and family counseling. The Case Manager acts as a liaison with sponsoring agencies, including utilization review.  Prepares accurate documentation of services delivered within set time frames.

Snapshot of Job Duties:

  • Evaluate each youth’s behavior according to the unit structure/expectations and the youth’s developmental capacity.

  • Review assessments of residents to determine emotional, physical, education, social, familial, and independent living needs of the resident.

  • Screen and assess students with Substance Abuse Diagnoses for the purpose of developing treatment plans.

  • Serve as the liaison with resident’s family, community case manager and community services in coordinating care.

  • Maintain quality codes of conduct and professionalism, i.e., strong work ethics, role modeling and commitment to improving quality of life to the youth in our care.

  • Attend staff / unit meetings to discuss facility and milieu operations and to receive facility communications.

  • Complete all documentation tasks related to the position. All documentation should be timely and accurate.

Schedule:  The days worked will be Monday through Friday between the hours of eight:30 am – 5:00 pm. This is a Full Time position with benefits.

Qualifications: This position requires a Masters in counseling or related field.  Experience working with adolescents is preferred but not required.
