Job Description



Purpose

Provide personal care services under the direction of the Director of Nursing. The Home Health Aide is assigned to specific clients and performs services for clients as necessary to maintain their personal comfort per the plan of care.



Essential Job Functions Duties and Responsibilities

  1. Perform personal care activities per specific client care instructions and the plan of care.
  2. Report any observed or reported changes in the client's condition.
  3. Promote personal safety and a safe environment for clients.
  4. Document care provided.
  5. Communicate effectively with all staff, clients and family members.
  6. Participate in staff meetings and team conferences, as requested.
  7. Attend in-service programs to meet company requirements.
  8. Maintain confidentiality at all times


Required Knowledge, Skills and Abilities

  • Must be at least eighteen (18) years of age.
  • Demonstrated ability to read, write, and follow a written Plan of Care.
  • Must possess the ability to make independent decisions and follow instructions.
  • Must be able to work well withtheill, disabled and theelderly.
  • Must be able to deal tactfully with clients, staff and family members.
  • Good verbal, written, and interpersonal skills.
  • Must be able to speak and write English in a manner that is sufficient for effective communication.


Education and Experience

  • Successful completion of a formal certification training program, including classroom and supervised practical training (minimum of 75 hours) and/or successful completion of competency exam.
  • Minimum of six (6) months of work experience in a homehealthcare agency.


Licensing/Certifications

Must qualify as a home health Aide in Florida with applicable training and/or competency evaluation requirements.
