We are currently hiring for C.N.A.s to complete home care visits in our senior client's homes. You will use your vehicle to drive to senior citizens homes and you will assist them with tasks they can no longer do by themselves and use your training. We pay for your mileage and driving time in between home visits.

Full-time home care hours available:

Must work every other weekend, working a shift between the hours of 8 am-8 pm Saturday and Sunday.

Monday through Friday working a split shift between the hours of 7 am and 9 pm. Split shifts are between 7-9 am or 7-10 am in the morning and 3-5 or 3-9 later in the day.

PRIMARY RELATIONSHIPS: Personal Care Service Manager, Personal Care Scheduler, Nursing Director, Home Care and Clinic staff, participants, family members and public.


OBJECTIVE: Under the direction and supervision of the Personal Care Service Manager responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to the preservation of the home environment or the Adult Day Health Center as needed. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior CommUnity Care. Functions as a member of Interdisciplinary Team.

PACE (Program of All-Inclusive Care for the Elderly) delivers all needed medical and supportive services to our participants. The program provides the entire spectrum of care and services to older adults with chronic care needs while maintaining their independence in their homes for as long as possible.

1. Consults each day with the Personal Care Service Manager and Personal Care Scheduler to be updated on Senior CommUnity Care participant’s status, changes in plan of care and to confirm scheduled visits.

2. Under the direction and supervision of the Registered Nurse assists participants in the home with the following duties:
a. Personal hygiene such as skin, mouth, hair care and bathing.
b. Assists participants in and out of bed assisting with ambulation.
c. Assisting with prescribed exercise which participants and aide have been taught by appropriate personnel.
d. Prepares meals, including therapeutic diets, assists participants with meals.
e. Assist participants to the bathroom or in using commodes, bedpans, or urinals.
f. Assisting participants with medications which are ordinarily self-administered.
g. Other duties taught by a health profession for a specific participant such as: taking of vitals, changing colostomy bags, changing of non-sterile dressing and non-sterile bowel and bladder hygiene care.

3. Performs directed household services.

4. Reports changes in participant’s condition and needs to the Personal Care Service Manager.

5. Completes appropriate documentation of services provided following Senior CommUnity Care policy.

6. Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.

7. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.

8. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

9. Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures.

10. Acts within the scope of his or her authority to practice.

11. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

12. Participates in and supports Quality Improvement Initiatives.

13. Performs other duties as required or requested.

Job Requirements

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

2. Must have a valid driver’s license, proof of insurance and have means of transportation.

3. Education: High School diploma or equivalent required. Current License as Certified Nursing Assistant (C.N.A) preferred.

4. Experience: A minimum of one year’s experience in working with the frail or elderly population required.

5. Skills and Knowledge:
• Knowledge of principles, practices standards and techniques of a C.N.A. or equivalent training
• Knowledge of common safety hazards and precautions to establish a safe work environment.
• Knowledge of medical equipment how to use and maintain.
• Skilled in establishing and maintaining effective working relationships with patients, co-workers, medical staff, and the public.
• Skilled in identifying and recommending problem resolution.
• Ability to work independently with minimum supervision.
• Ability to communicate clearly and effectively.
• Ability to react calmly and effectively in emergency situations.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

Physical Demands

1. Physical Requirements – Requires full range of body motion including handling and lifting patients. Requires standing, walking, pushing, bending, kneeling and reaching at arm’s length and overhead in clinic, rehab department and in community for prolonged periods of time. Must be able to transfer participants needing maximum assist of one. Requires the ability to lift/carry up to fifty (50) pounds and push or pull objects exceeding one hundred fifty (150) pounds, using appropriate body mechanics. Requires manual and finger dexterity and eye-hand coordination.

2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to document care provided in participants’ service records. Must be able to communicate verbally with participants in regard to their treatment plan. Communicate verbally with caregivers and co-workers.

3. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that may have the potential for verbal or physical aggression.

1. Follow correct policy and procedures for department and facility Fire/Disaster/Missing Person Plan. Know the location of fire alarms and extinguishers.
2. Follow correct policy and procedure for hand washing.
3. Follow guidelines for proper method of lifting.
4. All doorways, hallways and areas are to be kept clear.
5. Know the location and purpose of Material Safety Data Sheets (MSDS).
6. Follow guidelines for department environment and safety measures.
7. Follow guidelines for safe handling, inspection, maintenance and storage of equipment. Report any malfunction of equipment.
8. Follow guidelines for safe handling and storage of chemicals and supplies.
9. Follow correct policy and procedure for reporting of incidents: staff, residents, etc.
