Job Description



Nurse Supervisor, clients, family members, physicians and staff co-workers.


    The primary role of the Home Health Aide is to provide assigned residents with routine daily nursing care, activities of daily living, incidental homemaking and personal care in accordance with established agency nursing care procedures as directed by supervisors. The Home Health Aide reports any noted changes in the resident as they are noted.


    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.   • Have successfully completed a 75-hour training course and competency evaluation; or
  • Have successfully completed a competency evaluation approved by the Commissioner; or
  • Have successfully completed training in another jurisdiction equivalent to that required in 1 (a); or
  • Have successfully completed a training program or competency evaluation program which satisfies the requirements of Medicare for Home Health Aides as provided by the Code of Federal Regulations, Title 42, Section 484.36.
  • Have satisfied the following requirements:
    • Meet the requirements of Title XVIII of the Social Security Act for Nursing Assistants in nursing facilities certified for participation in the Medicare program or have successfully completed a Nursing Assistant training program approved by the State of Minnesota; and
    • Have had a least twenty (20) hours of supervised training or experience performing Home Health Aide tasks in a home setting under the supervision of a RN or complete the supervised practical training or experience within one-month after beginning work performing Home Health Aide tasks; or
  • Prior to June 1, 1993, had completed an approved Home Health Aide training program.
  • Be eighteen (18) years of age.
  • Have demonstrated sympathetic attitude toward care of the sick.
  • Process the ability to read, write and carry out directions.
  • Possess the maturity to deal effectively with the demands of the job.
  • Have demonstrated the ability to work with little supervision and make appropriate judgment.
  • Have demonstrated dependability, tact, and ability to follow orders.
  • Experience in an Assisted Living Facility or Memory Care Unit strongly preferred.
  • Ability to prioritize duties.
  • Customer service oriented.
  • Ability to understand and follow work assignments.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
  • Must be a team player.

Home Health Functions

  • Assists residents with personal care activities, including but not limited to:
    • Bathing – full or partial;
    • Shampoo/hair care;
    • Skin care/nail care, including foot soaks;
    • Oral hygiene/denture care;
    • Shaving;
    • Dressing.
  • Follows all shift routines, policy and procedures regarding accurate performance of vital signs, weights, etc.
  • Administer medications and treatments under the director and supervision of the RN (refer to Medication Administration section).
  • Assists with ambulation, transfers and exercise.
  • Assist with body positioning or transfers of clients who are not ambulatory.
  • Perform household services essential to health care at home, including, but not limited to:
    • Meal preparation/feeding;
    • Laundry;
    • Light housekeeping.
  • Assist with bowel and bladder control, devices and training programs.
  • Respond and attend to client requests promptly.
  • Maintain proper hand washing techniques.
  • Maintain a safe client environment.
  • Maintain client confidentiality; treat clients and families with respect.
  • Understand, accept and respond to emotional needs of each client.
  • Reports promptly to the nurse any changes in the resident condition.
  • Reports any indication of resident illness to the nurse.
  • Provides any information on changes in skin conditions to the nurse.
  • Observe and report any noted changes in mental status to the nurse.
  • Report to charge nurse all incidents, accidents that occur involving residents, and visitors.
  • Follow standard precautions and works within OSHA guidelines.
  • Treats and ensures that residents are treated with respect and dignity.
  • Immediately reports any suspected abuse or neglect per facility policy.
  • Remains aware and takes special precautions to ensure a clean safe environment for residents and staff.
  • Reports any equipment malfunction to designated employee.
  • Accept and fulfill assignments with the Agency; exercise judgment in accepting assignments.
  • Perform other related duties and responsibilities as deemed necessary.


  • Accurate documentation of cares given and pertinent observations.

    Personnel Functions

    • Attend designated home health meetings as scheduled.
    • Develop and maintain a good working relationship with inter-departmental personnel.
    • Attends educational offerings and obtains at least 12 hours of continuing education a year per regulation.

    Committee Functions
  • Serve on, participate in and attend various committees and meetings of the facility to enhance interdisciplinary process.

Job Requirements

Medication Administration Procedure

  • A Home Health Aide may administer medications, whether oral, suppository, eye drops, ear drops, inhalant, topical or administered through a gastrostomy tube if:
    • The medications are regularly scheduled.
    • Prior to the administration, the Home Health Aide is instructed by a RN in the procedures to administer the medications to each client.
    • A RN specifies, in writing, and documents in the client’s clinical records, the procedures to administer the medications to each client.
    • Prior to the administration, the Home Health Aide demonstrates to a RN their ability to competently follow the procedure. This competency shall be documented by the RN and placed in the Home Health Aide’s personnel file.
    • In the case of pro re nata (PRN) medications, the administration of the medication is reported to a RN either:
      • Within twenty-four (24) hours after its administration; or
      • Within a time period that is specified by a RN prior to the administration.
  • PRN medications shall be defined as a medication that is ordered to be administered or taken by a client as necessary. A Home Health Aide may not administer or inject medications into veins, muscle or skin.

Performance of Routine Procedures

  • A Home Health Aid may perform delegated medical or nursing and assigned therapy procedures if:
  • Prior to performing the procedures, the Home Health Aide is instructed by a RN or Therapist in the proper methods to perform the procedures with respect to each client.
  • A RN or Therapist specifies, in writing, specific instructions for performing the procedures for each client.
  • Prior to performing the procedures, the Home Health Aide demonstrates to a RN or Therapist the Home Health Aide’s ability to competently follow the procedures.
  • The procedures for each client are documented in the clinical record.

  • Ability to endure prolonged walking, sitting, standing, use of pulling, bending, and stooping movements.

  • Ability to perform tasks which require arm-hand steadiness.
  • Ability to walk and stand for prolonged periods of time through duration of shift(s)
  • Ability to squat, climb stairs, reach above shoulders, twist, bend and kneel repeatedly through duration of shift(s).
  • Ability to see and interpret colors as in reading warning lights.
  • Ability to hear normal sounds within background noise.
  • Ability to speak clearly and make self understood.
  • Ability to keep attention on task if routinely interrupted.
  • Ability to understand a variety of concepts/approaches.
  • Ability to remember tasks/assignments for a full shift
  • Ability to work beyond regularly scheduled shift (overtime) when necessary.
  • Ability to adapt to changes in daily work hours and schedule.
  • Must be able to push/lift up to one hundred (100) pounds.

Employees will be expected to:

  • Follow correct policy and procedures for department and facility Fire/Disaster/Missing Person Plan. Know the location of fire alarms and extinguishers.
  • Follow correct policy and procedure for hand washing.
  • Follow guidelines for proper method of lifting.
  • All doorways, hallways and areas are to be kept clear.
  • Know the location and purpose of Safety Data Sheets (SDS).Follow guidelines for department environment and safety measures.
  • Follow guidelines for safe handling, inspection, maintenance and storage of equipment. Report any malfunction of equipment.
  • Follow guidelines for safe handling and storage of chemicals and supplies.
  • Follow correct policy and procedure for reporting of incidents: staff, residents, etc.
