We are looking for a full time LPN to join our team in a rewarding but fast paced environment at our Senior CommUnity Care PACE Center!

At our Senior CommUnity Care PACE Center, we provide health care in the home and community to older adults who would otherwise live in nursing homes. You will work primarily in either our Day Center or Home Health but will need to be flexible to do both. PRIMARY RELATIONSHIPS: Clinic Supervisor, Center Director, Executive Director, Center staff, participants, family members and public.

OBJECTIVE: Under the direction and supervision of the Clinic Supervisor, uses and practices basic assessment, participates in planning, executes interventions in accordance with the plan of care and contributes to the evaluation of individualized interventions related to the plan of care. Administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone. Provides education to Senior CommUnity Care participant and caregiver in regards to plan of care, nursing treatments or medications. Communicates participant’s condition to primary care givers, physician and interdisciplinary team.


1. Uses and practices basic assessment and assists Clinic Supervisor as designated to complete initial assessment of new PACE enrollee.

2. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.

3. Uses and practices basic assessment and assists Clinic Supervisor as designated to complete annual reassessment.

4. Performs basic nursing services as delegated by Clinic Supervisor.

5. Performs the administration of prescribed medications and treatments in accordance with nursing standards.

6. Maintains participants’ medical record and fulfills Senior CommUnity Care charting and reporting requirements.

7. Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians.

8. Triages participants to determine priority of care.

9. As delegated, assists Clinic Supervisor or Clinic Registered Nurse (R.N.) when necessary in signing off orders, ordering medications from pharmacy, and disseminating orders and instructions to appropriate personnel. Implements the orders written by primary care and nursing orders when necessary.

10. Instructs participants and family regarding medications and treatment instructions and care plan.

11. Provides education and training on care and treatment issues of the frail and elderly as requested.

12. Participates in administering participant vaccinations and Tuberculosis (TB) testing. Will document and record appropriately.

13. Prepares equipment and aids physician during treatment, examination, and testing of participants.

14. Assists other staff in clinic in daily appointments and emergencies. Maintains timely flow of participants.

15. Responds to and refers incoming telephone calls.

16. Assists in the maintenance of the exam rooms for necessary supplies and materials. Ensures cleanliness. Lists needed clinic supplies on supply list as appropriate.

17. Effectively communicates in Interdisciplinary Team meetings, clinic meetings or any other meetings as assigned.

18. Assists in the scheduling of specialist appointments and assists in hospital admissions and calling report to unit, as assigned or requested

19. Acts within the scope of his or her authority to practice.

20. Follows all Senior CommUnity Care Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.

21. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.

22. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

23. Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures.

24. Participates in and supports Quality Improvement Initiatives.

25. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

26. Performs other duties as required or requested.


Job Requirements

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. 1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

2. Must have a valid driver’s license, proof of insurance and have means of transportation.

3. Education: Graduate of a school of professional nursing required. Current Colorado Practical Nurses License (L.P.N.), Cardio Pulmonary Resuscitation (CPR) certification required.

4. Experience: A minimum of one year’s experience in working with the frail or elderly population required. Intravenous Venous (IV) Certification preferred.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individual with disabilities to meet these demands:

1. Physical Requirements – Requires full range of body motion including handling and lifting patients. Requires standing, walking, pushing, bending, kneeling and reaching at arm’s length and overhead in clinic, rehab department and in community for prolonged periods of time. Must be able to transfer participants needing maximum assist of one. Requires the ability to lift/carry up to fifty (50) pounds and push or pull objects exceeding one hundred fifty (150) pounds, using appropriate body mechanics. Requires manual and finger dexterity and eye-hand coordination.

2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to document care provided in participants’ service records. Must be able to communicate verbally with participants their treatment plan, team members and participants caregivers.

3. Pressure Factor - Requires working under occasional potential stressful conditions. May be exposed to working conditions that could be noisy, crowded and with fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants.

1. Follow correct policy and procedures for department and facility Fire/Disaster/Missing Person Plan. Know the location of fire alarms and extinguishers.
2. Follow correct policy and procedure for hand washing.
3. Follow guidelines for proper method of lifting.
4. All doorways, hallways and areas are to be kept clear.
5. Know the location and purpose of Material Safety Data Sheets (MSDS).
6. Follow guidelines for department environment and safety measures.
7. Follow guidelines for safe handling, inspection, maintenance and storage of equipment. Report any malfunction of equipment.
8. Follow guidelines for safe handling and storage of chemicals and supplies.
9. Follow correct policy and procedure for reporting of incidents: staff, residents, etc.
