Tivoli Manor also sits on the campus and provides the much needed service of Memory Support Assisted Living.

We are seeking a MDS/Care Plan/Case Management RN for our skilled nursing facility.  The ideal candidate is compassionate, caring and reliable to join our team.

OBJECTIVE:  The primary purpose of the MDS Nurse is to plan and organize the MDS/RAI process in coordination with the RAI/PPS Coordinator.  This position assures compliance with all State and Federal MDS transmission requirements.  The MDS Nurse is responsible for collaborating with Clinical and Administrative staff regarding issues relative to the Resident Assessment Instrument (RAI) process.

ESSENTIAL FUNCTIONS: 
A.    Resident Assessment Instrument Process
B.    Uses Quality Indicator data and other reports to improve MDS accuracy & compliance
C.    Personnel Functions
D.    Other duties as assigned

Job Requirements

QUALIFICATIONS:  
1.    Current licensure in the state of employment as a Registered Nurse.
2.    Registered Nurse with previous long- term care experience with working knowledge of the RAI process preferred.
