Volunteers of America, Durham, NC, is looking for an RN to join it's team! 

The Nurse Case Manager provides case management, coordination, supervision and implementation of professional and supportive services to participants/clients using nursing theory and process in accordance with the Nurse Practice Act.  

CORE RESPONSIBILITIES:  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill and/or ability required.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

1.    Minimum of two (2) years of nursing experience, with a combination of long-term care dementia care or acute care.  Home care experience preferred.
2.    Twenty-one (21) years of age, meet the MVR organization standards, and have a valid driver’s license in the state the facility is located in.
 
ESSENTIAL FUNCTIONS: triage, perform clinical procedures, blood draws, administer medications by all routes, place follow up calls and organize external appointments to specialist along with other duties assigned by Clinic Manager.

Job Requirements

 

  1. Current licensure in the state of employment as a Registered Nurse.
  2. Current CPR/AED/First Aid Certification.
  3. Maintenance of required CEU’s.
  4. Experience working with geriatrics preferred.
  5. Good problem solver with abilities to make independent judgments utilizing initiative, discretion and the nursing process as needed.
  6. Demonstrates leadership ability.
  7. Effective communicator with staff, families and outside agencies.
  8. Ability to prioritize duties.
  9. Customer service oriented with knowledge of successful customer service techniques.
  10. Clinical competence to utilize the nursing process to perform physical and cognitive assessments of residents.
  11. Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
  12. Must be a team player.
  13. Understanding of current Antibiotic Stewardship, Infection Prevention and Infection Control and OSHA guidelines.
  14. Must be a team player.
