We are looking for a full time Registered Nurse (RN) to join our team in a rewarding but fast paced environment at our Senior CommUnity Care PACE Center!

At our Senior CommUnity Care PACE Center, we provide health care in the home and community to older adults who would otherwise live in nursing homes. You will work primarily in either our Day Center or Home Health but will need to be flexible to do both. Under the direction and supervision of the Nurse Manager, the RN provides skilled nursing care to participants, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family teaching and communicates participant’s condition to primary caregivers, physician and Interdisciplinary Team.

  1. Conducts an in person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant. 
  1. Conducts an in person nursing re-assessment semi-annually and as needed. 
  1. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery. 
  1. As needed provides orientation, initial and annual competencies, of, Licensed Practical Nurse (LPN), Certified Nursing Assistant’s (C.N.A.’s), and Clinic Clerk. 
  1. Performs competencies on appropriate day center staff as needed. 
  1. Performs general nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards. 
  1. Triages participants to determine priority of care. 
  1. Signs off orders, orders medications from pharmacy and disseminates orders and instructions to appropriate personnel (call orders to nursing homes, etc.). 
  1. Working with Nurse Manager, completes the nursing assessment form on visitors and on participants at assigned schedule. 
  1. Effectively communicates in Interdisciplinary Team meetings, family meetings, and clinic meetings. 
  1. In Nurse Manager’s absence, supervises LPN’s and C.N.A.’s. Evaluates overall effectiveness of the clinic. 
  1. Assists in the treatment, examination, and testing of participants.
  1. Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians. 
  1. Working with the Nurse Manager, oversees appointment bookings and ensures preferences are given to participants in emergency situations. Maintains timely flow of participants. 
  1. Responds to and performs triage of incoming telephone calls. Schedules outside appointments for participants when necessary. 
  1. Implements the orders written by Primary Care Physician. 
  1. Schedules specialist appointments and assists in hospital admissions and calling report to unit. 
  1. Instructs and educates participants and family regarding medications and treatment instructions. 
  1. Maintains and reviews participant records, charts, and other pertinent information. Documents test and examination results. 
  1. Ensures that exam rooms are stocked for necessary supplies and materials. Ensures cleanliness.  Ensures that appropriate staff maintains a list of medical supplies. 
  1. Acts within the scope of his or her authority to practice. 
  1. Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines. 
  1. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions. 
  1. Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures. 
  1. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families. 
  1. Participates in and supports Quality Improvement Initiatives. 
  1. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications. 
  1. Takes call after hours as scheduled in the nursing department. 
  1. Performs other duties as required or requested.

Job Requirements

  1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. 
  1. Must have a valid driver’s license, proof of insurance and have means of transportation. 
  1. Education: Graduate of a school of professional nursing.  Current State Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred. 
  1. Experience: A minimum of one year’s experience in working with the frail or elderly population required.
  1. Skills/Knowledge:
  • Broad knowledge base of health care problems of the frail elderly.
  • Knowledge of medical equipment and instruments.
  • Knowledge of common safety hazards and precautions to establish a safe working environment.
  • Skilled in identifying problems and recommending solutions.
  • Skilled in preparing and maintaining records and writing reports, and responding to correspondence.
  • Skilled in developing and maintaining department quality assurance.
  • Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
  • Ability to react calmly and effectively in emergency situations.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation
