At our Senior CommUnity Care PACE Center, we provide health care in the home and community to older adults who would otherwise live in nursing homes. You will work primarily in either our Home Health but will need to be flexible to do work in the Day Center as well. Our goal is to continue to help people live independently at home with a high quality of life - come join us!

PRIMARY RELATIONSHIPS: Clinic Supervisor, Center Manager, Executive Director, Center staff, participants, family members and public.

OBJECTIVE: Under the direction and supervision of the Clinic Supervisor, provides skilled nursing care to participants, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family teaching and communicates participant’s condition to primary caregivers, physician and Interdisciplinary Team.

ESSENTIAL FUNCTIONS:

1. Conducts an in person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.

2. Conducts an in person nursing re-assessment semi-annually and as needed.

3. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.

4. As needed provides orientation, initial and annual competencies, of, Licensed Practical Nurse (LPN), Certified Nursing Assistant’s (C.N.A.’s), and Clinic Clerk.

5. Performs competencies on appropriate day center staff as needed.

6. Performs general nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards.

7. Triages participants to determine priority of care.

8. Signs off orders, orders medications from pharmacy and disseminates orders and instructions to appropriate personnel (call orders to nursing homes, etc.).

9. Working with Clinic Supervisor, completes the nursing assessment form on visitors and on participants at assigned schedule.

10. Effectively communicates in Interdisciplinary Team meetings, family meetings, and clinic meetings.

11. In Clinic Supervisor absence, supervises LPN’s and C.N.A.’s. Evaluates overall effectiveness of the clinic.

12. Assists in the treatment, examination, and testing of participants.

13. Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians.

14. Working with the Clinic Supervisor oversees appointment bookings and ensures preferences are given to participants in emergency situations. Maintains timely flow of participants.

15. Responds to and performs triage of incoming telephone calls. Schedules outside appointments for participants when necessary.

16. Implements the orders written by Primary Care Physician.

17. Schedules specialist appointments and assists in hospital admissions and calling report to unit.

18. Instructs and educates participants and family regarding medications and treatment instructions.

19. Maintains and reviews participant records, charts, and other pertinent information. Documents test and examination results.

20. Ensures that exam rooms are stocked for necessary supplies and materials. Ensures cleanliness. Ensures that appropriate staff maintains a list of medical supplies.

21. Acts within the scope of his or her authority to practice

22. Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.

23. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

24. Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures.

25. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.

26. Participates in and supports Quality Improvement Initiatives.

27. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

28. Performs other duties as required or requested.

 

Job Requirements

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. 1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

2. Must have a valid driver’s license, proof of insurance and have means of transportation.

3. Education: Graduate of a school of professional nursing. Current Colorado Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred.

4. Experience: A minimum of one year’s experience in working with the frail or elderly population required.


5. Skills/Knowledge:
• Broad knowledge base of health care problems of the frail elderly.
• Knowledge of medical equipment and instruments.
• Knowledge of common safety hazards and precautions to establish a safe working environment.
• Skilled in identifying problems and recommending solutions.
• Skilled in preparing and maintaining records and writing reports, and responding to correspondence.
• Skilled in developing and maintaining department quality assurance.
• Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
• Ability to react calmly and effectively in emergency situations.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individual with disabilities to meet these demands:

1. Physical Requirements – Requires a full range of motion including handling and lifting patients. Requires frequent standing, walking, pushing, bending, kneeling and reaching at arms length and overhead in clinic, rehab department and in community for prolonged periods of time. Must be able to transfer participants needing maximum assist of one. Requires the ability to lift/carry up to fifty (50) pounds and push or pull objects exceeding one hundred fifty (150) pounds, using appropriate body mechanics. Requires manual and finger dexterity and eye-hand coordination.

2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to document care provided in participants’ service records. Must be able to communicate verbally with participants their treatment plan, team members and participants caregivers.

3. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that may have the potential for verbal or physical aggression.

4. All doorways, hallways and areas are to be kept clear.
5. Know the location and purpose of Material Safety Data Sheets (MSDS).
6. Follow guidelines for department environment and safety measures.
7. Follow guidelines for safe handling, inspection, maintenance and storage of equipment. Report any malfunction of equipment.
8. Follow guidelines for safe handling and storage of chemicals and supplies.
9. Follow correct policy and procedure for reporting of incidents: staff, residents, etc.
10. Ability to communicate in English including reading, writing and speaking on the phone.
