Job Description

Traveling Float Social Worker, Locum work though out the Volunteers of America National Services Elder Care communities Nation Wide. 

Social Worker who is travel savvy and engaging with Elder population. Volunteers of America National Services has locations across the country. MN, CO, FL, AZ, NV. You would travel 90% of your work life. Spending long and short term assignments at various locations. 

To support in the care and treatment of clients entrusted to the agency. To serve as a liaison and role model to the community for the agency. Represents the Social Service department, other care center staff and the agency as a whole to the community. Upholds the values and mission of Volunteers of America National Services to peers and the public. Customers include clients of the agency and their families, local healthcare professionals who work with the agency and staff, and the agency staff managers.


  • Performs social services assessment, counseling, education, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care. Completes assessments of clients as necessary for Care Planning, MDS, RAPS, PPS and other required documents.
    • Completes assessments in a timely manner and in accordance with agency policy.
    • Develops a written plan to provide information and direction to other health team members.
    • Documents all findings, plans, interventions, and client progress in the clinical record.
    • Communicates plans and changes to the physician and/or nursing and other agency staff through the care plan, written progress notes, and participation in care conferences.
  • Prepares social histories to augment existing service, or as a guide in determining or changing level of service, taking into consideration:
    • Family relationships
    • Meaning of illness to the client and his/her family
    • Cultural attitudes in relationship to illness
    • Meaning of “home” to client
    • Ability of client and family to adjust to the care of the client in the home
    • The financial impact of client’s illness upon the family
  • Develops Care Plans and attends Interdisciplinary Care Conferences for clients.
    • Serves as client advocate.
    • Coordinates invitation of clients and families.
    • Refers to Volunteers of America National Services Care Planning and MDS manuals to complete required sections.
    • Implements described case management, counseling or support described in Care Plan.
    • Maintains confidential records as necessary.
  • Initiates reporting and documentation in the medical record.
    • Maintains accurate and appropriate social service documentation in the medical record.
    • Completes and updates assessments and documentation per Volunteers of America National Services social work policy and State/Federal regulations.
    • Prepares written summaries of conferences and meetings as necessary.
    • Notifies admissions coordinator of payer change status for daily census.
  • Develops and promotes the department through marketing efforts throughout the professional and social community.
    • Maintains a current Community Resource File.
    • Visits referral agencies and local hospitals to share information on agency.
    • Provides community resource information to clients and families relating to alternative health care services or facilities, clinics, etc.
    • Plans and presents sessions to community and professional groups on the agency and on the problems of aging and adjusting to long-term care agency life.
  • Serves on committees as required by Administrator.
  • Serves as rights advocate liaison to the agency; coordinates posting of all Federal and State required material for client and family advocacy and updates materials as necessary.
  • Advises the Administrator of major problems or concerns of clients or their family members; recommends solutions.
  • Ensures that clients have necessary and adequate clothing, health aids, etc.
  • Performs other duties or special assignments as directed by the Administrator.

Job Requirements


  • Bachelor’s degree in Social Work or related degree in social sciences; licensed where applicable by the State; State requirements may dictate supervision and degree type.
  • Minimum of one-year of Social Work experience, in a healthcare setting; knowledge of home health care and public health settings; familiarity with Federal and State regulations and requirements for home health care.
  • Twenty-one (21) years of age, meet the MVR organization standards, and have a valid state driver’s license.
  • Ability to read, speak and write fluently in English; excellent communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules and regulations; carry on negotiations for client’s welfare on the phone.
  • Ability to add, subtract, multiply and divide in all units of measure using whole numbers, common fractions and decimals; ability to comprehend and execute simple statistical functions to create standard graphs and tables.
  • Ability and willingness to work flexible hours, such as during evenings and weekends, for planned activities or special events, family meetings and conferences and admission calls.
  • Negotiation skills and ability to carry on several levels of reasoning at once during problem solving sessions; ability to think quickly and to write contracts and complex written arguments.
  • Ability to work with clients of varying levels of responsiveness.
  • Familiarity with word processing, data processing and computer entry skills to match existing clinical management software at agency.
  • Ability to perform several job functions at once.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
