Join a team of competent and caring professionals as a Nurse Care Manager at Behavioral Health Network’s BHCP program in Holyoke, MA! Through their organized nature and attention to detail, Nurse Care Managers use their problem-solving skills to identify the needs of individuals and discover solutions for the healthcare team to meet those needs. The Nurse Care Manager uses their broad knowledge of sound clinical theories and principles to provide high quality clinical care management services and consultation.

The Nurse Care Manager is team-based, making sure that all appropriate members in the Integrated Care Team are included in decision-making when they implement and review comprehensive, person-centered Integrated Care Plans. They also ensure positive health outcomes and prudent use of resources by monitoring the effectiveness of the Integrated Care Plans and making adjustments as needed. Collaborating and consulting with internal and external service providers, the Nurse Care Manager makes sure to meet the needs of the individuals we serve timely and efficiently while maintaining direct service productivity.

Behavioral Health Community Partners (BHCP) team at BHN has extensive experience in managing and coordinating care for adults ages 21-64. It holds each individual at the core of what it does. Through the coordination and integration of care, BHCP assists individuals with medical, behavioral health, or substance abuse concerns in navigating the health care system and improving their health outcomes.

Job Requirements

Nurse Care Managers at Behavioral Health Network have the following qualities:

  • Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management. 
  • Familiarity with internet, word processing and emails systems.
  • RN License (required).
  • Associate’s Degree in Nursing (required).
